As the owner of WH Bike Fit in North Carolina, Wendy combines her deep understanding of human movement with a personalized, hands-on approach to bike fitting. She works closely with coaches and trainers to ensure that competitive athletes optimize performance and that recreational riders enjoy their time on the bike—whether training for races or cruising on coffee rides. Wendy has worked with elite athletes and teams, including UAE Team Emirates and events like the ABSA Cape Epic, bringing world-class expertise to every fit she performs. Pelvic complaints among cyclists are well-documented, affecting both men and women due to prolonged pressure on the perineum and pelvic region. Studies indicate that high-intensity cycling can lead to urological and sexual disorders, including pelvic pain, genital numbness, and recurrent urinary infections in women (Giraldo Gómez et al., 2024). Female cyclists, particularly those engaged in competitive or long-distance cycling, may experience conditions such as "bicyclist's vulva," a chronic unilateral swelling of the labium majus caused by repeated pressure and lymphatic dysfunction (Baeyens et al., 2002). Additionally, research highlights that female riders suffer from decreased genital sensation, potentially contributing to sexual dysfunction, though studies remain inconclusive in establishing a direct causal relationship (Partin et al., 2014). Ergonomic factors, such as saddle design and handlebar height, play a crucial role, as handlebars positioned lower than the saddle have been associated with increased perineal pressure and reduced genital sensation in women (Partin et al., 2012). While gender-specific cycling equipment may help mitigate some of these risks, more research is needed to develop effective preventive strategies and interventions. A specialized pelvic health physiotherapist can play a crucial role in addressing cycling-related pelvic complaints by assessing musculoskeletal, neurological, and postural factors contributing to pain, numbness, and dysfunction. They can provide individualized interventions, including postural and bike fit adjustments to optimize saddle positioning, handlebar height, and overall cycling ergonomics, reducing perineal pressure. Pelvic floor muscle training can help cyclists experiencing pelvic pain, urinary incontinence, or sexual dysfunction by improving strength, flexibility, and coordination of these muscles. Additionally, manual therapy and soft tissue release techniques may be used to alleviate muscle tension, improve circulation, and reduce nerve compression in the perineal region. Physiotherapists can also employ nerve mobilization techniques to address pudendal nerve entrapment, which is a common issue in cyclists experiencing numbness or burning sensations. Education on saddle choice and cycling hygiene is essential, especially for female cyclists prone to conditions like "bicyclist's vulva," as proper gear and positioning can prevent lymphatic dysfunction. Lastly, physiotherapists may incorporate core and hip stability exercises to improve overall pelvic alignment and reduce excessive loading on sensitive areas.
