A broad white descending diagonal separating a yellow field in the upper fly from a dark green field in the lower hoist, with a central white disk bearing the municipal arms.

In this variant, the colours are reversed, and the white central disk is omitted.

Neither of these two variants seems to match the flag in use in Coronel Sapucaia. On photos of the flag taken in the Mayor's office, the diagonal stripe is grey with the coat of arms inscribed in what seems to be a white square diamond, not overlapping the colored triangles:
