One sequence depicts a character caressing and straddling a man in bed. Additionally, there are references to prostitution. Sexual moaning sounds can be heard in a brothel. Even though the game contains no explicit nudity, characters breasts and buttocks are also seen partially exposed.

Players use swords and magic attacks to defeat enemies in melee-style combat. Blood splatter effects occur frequently during these sequences, and some cutscenes also depict additional acts of violence and bloody images: this includes a character's hands being dismembered by a sword, a guard having his throat slit, a character being impaled through the chest, and a prisoner's shoulder stabbed by an interrogator's blade.
All instances of graphic violence occur in Cutscenes. During gameplay, violence is surprisingly tame in comparison. Blood spurts appear in most occasions when characters are struck but due to the flashy visual effects they are extremely easy to miss.
This is probably the single most violent Final Fantasy game to date, though in terms of medieval fantasy games, it doesn't really hold a candle to the likes of The Witcher 3 and others, there's rarely any gore and mostly blood instead, though little is left up to the imagination either.
The very first bit of violence you see when starting a new game is a combat sequence between Phoenix and the Infernal Eikon, and goes to explain how the game is bloodier and gorier than the previous mainline installment.

Frequent uses of "Shit" and other words frequently heard in other FF titles.
This is the first mainline Final Fantasy title to use "fuck" in dialogue. However, despite the game taking advantage of the M rating, the word is used rather infrequently.

This is easily the darkest Final Fantasy game yet, with many dark themes and shocking moments, as well as an increase in violence, sexual themes, and even swearing, not to mention a very morally ambiguous lead and a world and plot that get progressively darker as it goes along.
A character commits suicide after a tragedy. The scene is quite sad and for those with experience with the subject it may be a bit intense.
If this game were to be rated by the MPA it would be rated R for strong violence, bloody images, including depictions of suicide, sexual references, partial nudity and language throughout.
