Great speaking with Florence Vuichard and Pascal Michel for a story in today's "Schweiz am Wochenende". We talked about medicines people urgently need and sometimes cannot get, because current supplies are in different pack sizes than prescribed or because they are warehoused in different countries from which they cannot be imported due to regulatory hurdles. In my view there would be an easy solution: liberalilze the pharmaceutical supply chain accross borders and agree on prices that reflect the true value of essential generic medicines. We also spoke about the medicines everybody wants but nobody can produce enough of: weight-loss drugs. While this is a unique market situation like I have never seen before, we also need address rising environmental concerns with a dramatically growing number of one-time application devices.

2024: Coming to end. When I reflect back, I could see that its been a great year for me , in every possible way. Develop M3: Each one of them have their unique role to play and add their own strength to your goal and strategizing them. Let me explain first their components: 1)  Mind : Includes knowledge, Information, strategy 2)  Muscle: Physical, Emotional, Mental , Allies, Social , Political 3)  Money: Visible, Invisible, Tangible, Non tangible Stay away from "I" Quattro people : Never wrestle with a pig. You'll both get dirty but only the pig will enjoy it 1) Irresponsible 2) Illogical 3) Irrational and 4) Most importantly IDIOTS Focus on T3 1) Time: Its finite and slipping away. You cannot save it, multiple it, store it , buy it. You just use it every second. Its the most precious commodity in life so use it wisely 2) Task: Prioritize the tasks. What needs to be done has to be done. What is not important can wait 3) Threats: Life is a threat at each moment. Life is like crossing the road blindfolded, you don't know what will hit you when. So be prepared for worst all the time: Emotionally, Mentally, Physically, Psychosocially, intellectually and Family wise.

As part of this phenomenon there is a growing focus on providing patients with greater flexibility with where and when they receive care. It is important to stress that this movement is not merely looking to increase at-home delivery of medicines, but instead to introduce structural changes to healthcare systems, focusing on treating patients outside of the hospital where feasible. 𝐑𝐨𝐜𝐡𝐞 𝐭𝐨 𝐟𝐚𝐬𝐭-𝐭𝐫𝐚𝐜𝐤 𝐰𝐞𝐢𝐠𝐡𝐭-𝐥𝐨𝐬𝐬 𝐩𝐢𝐥𝐥 𝐭𝐨 𝐜𝐨𝐦𝐩𝐞𝐭𝐞 𝐰𝐢𝐭𝐡 𝐫𝐢𝐯𝐚𝐥𝐬 Roche has announced plans to accelerate the production of a new weight-loss pill to vie with competitors like Eli Lilly and Novo Nordisk in the expanding obesity treatment market. Thomas Schinecker, CEO of Roche, conveyed to the Financial Times that the company anticipates its inaugural obesity drugs to hit the market much sooner than expected, potentially as early as 2028. The forthcoming offerings include a weight-loss injection slated for phase II trials and a pill that demonstrated a 6.1% weight reduction relative to a placebo over a four-week period. Following the acquisition of biotech firm Carmot last year for up to $3.1 billion, Roche is set to introduce several drugs under this deal, with details revealed for three key assets. Schinecker mentioned that the acquisition could yield approximately seven drugs, some still in early developmental stages. Last week's data publication regarding the pill caused Roche's stock to surge by 6% on that day, while impacting the shares of rival companies Novo Nordisk and Eli Lilly, raising concerns about Roche’s potential to challenge their market dominance. Analysts at Goldman Sachs have projected that the obesity drug market could exceed $130 billion by 2030, as various companies aim to launch their treatments. Despite this, Roche must bridge the gap with frontrunners who are developing more potent and effective treatments. For instance, Novo Nordisk’s Wegovy and Eli Lilly’s Mounjaro have led to 15% and 20% weight losses, respectively, in trials extending beyond a year. Other pharmaceutical firms, including Boehringer Ingelheim and Pfizer, are also in the race to develop obesity drugs, while US biotech Viking Therapeutics saw a 30% jump in shares after announcing progress to late-stage trials for its obesity pill. Schinecker highlighted that Roche’s pill is easier to scale up as it is synthetically produced, unlike the peptide-based drugs like those from Novo Nordisk. However, Emily Field from Barclays noted that it’s premature to determine if Roche can disrupt the advantage held by Novo and Lilly. Roche has recently streamlined its drug development pipeline, cutting 25% of underperforming drugs and focusing on a smaller selection of promising candidates, including those for obesity and Alzheimer’s diseases. This strategic shift allows for better allocation of resources towards more promising projects. Despite recent setbacks in research, including a failed lung cancer drug trial, Roche remains optimistic about its obesity drugs, potentially integrating them with other treatments for related conditions. Additionally, strong sales from other drugs like Vabysmo and Ocrevus have led Roche to upgrade its 2024 earnings forecast to the high single-digit range, up from the previously anticipated mid-single digit increase. In today’s fast-paced business world, nearly every major corporation has embarked on transformation programs, but sadly, only 12% achieve lasting success. In their insightful article "Transformations that Work" in the May-June edition of Harvard Business Review, authors Michael C. Mankins and Patrick Litre describe their six key practices that set successful transformations apart: ✅ treating transformation as an ongoing process ✅ integrating it into daily operations ✅ managing organizational energy wisely ✅ replacing benchmarks with aspirations when it comes to goal setting ✅ driving transformation programs from the "middle out" rather than top-down  ✅ and securing substantial external capital right form the start. Brilliant panel discussion on "Investments in Global Healthcare Systems" moderated by André Guedel at #swissbiotechday2024. It effectively highlighted the pivotal role of the Swiss life sciences industry, in collaboration with Swiss government organizations, academic institutions, and Swiss-based non-profit organizations, in shaping healthcare programs worldwide. Especially in Low-Middle-Income Countries #LMICs, #value-based partnerships have a transformative impact on patient access to healthcare. 💡The discussion covered a range of strategies, from funding financial sector partners to develop and scale health insurance products, enabling patients to afford therapies out-of-pocket #SCBF, to collaborating with local governments and organizations like #WHO AFRO to streamline national treatment guidelines and regulatory systems #swissmedic #novartis. European regulatory authority adopts a positive opinion for an update of the Wegovy® label to reflect risk reduction of major adverse cardiovascular events Bagsværd, Denmark, 25 July 2024 – Novo Nordisk today announced that the European Medicines Agency’s (EMA) Committee for Medicinal Products for Human Use (CHMP) has adopted a positive opinion for an update of the Wegovy® (semaglutide 2.4 mg) label to reflect data from the SELECT cardiovascular outcomes trial, demonstrating a risk reduction of major adverse cardiovascular events (MACE) including cardiovascular death, non-fatal heart attack (myocardial infarction) or non-fatal stroke in adults with established cardiovascular disease (CVD) and either overweight or obesity (initial BMI ≥27 kg/m2) without diabetes.

Today, I have been part of the NextGen Obesity Panel discussion at the Sachs Associates Annual Biotech in Europe Forum in Basel, Switzerland. Key reflections from the discussion it that currently we are treating obesity as it is one disease. But it is not! Obesity is a complex multifactorial disease that involves the interplay between genes, epigenetics, metabolism, and behavior. Therefore, we need more tools in the toolbox of anti-obesity medications. The future of anti-obesity therapy should focus on the following: ❇️ Novel drug concepts: To harness the full weight loss potential of weight loss medications there is a need for novel drug concepts – either as novel stand-alone alternatives to incretins or as add-on therapies to improve weight loss efficacy and/or associated comorbidities (e.g. diabetes). There are many interesting combinations approaching. ❇️ Muscle mass preservation: With current weight loss strategies lean mass accounts for 20-40% of the weight lost. The next generation of anti-obesity therapy would ideally conserve or even ad muscle during weight loss! ❇️ Sustained body weight reduction: This is really the true “holy grail” of obesity pharmacology, can we prevent weight rebound after stop of current treatments! It is impressive how far the obesity field has moved within a decade with treatment options now providing clinically relevant weight loss. It is also clear that we are not done yet. It will be exciting to see where the next decade will take us!
