Abstract
Photobiomodulation is reported to positively influence hair regrowth, wound healing, skin rejuvenation, and psoriasis. Despite rapid translation of this science to commercial therapeutic solutions, significant gaps in our understanding of the underlying processes remain. The aim of this review was to seek greater clarity and rationality specifically for the selection of optical parameters for studies on hair regrowth and wound healing.
Our investigation of 90 reports published between 1985-2015 revealed major inconsistencies in optical parameters selected for clinical applications. Moreover, poorly understood photoreceptors expressed in skin such as cytochrome c oxidase, cryptochromes, opsins, may trigger different molecular mechanisms. All this could explain the plethora of reported physiological effects of light.
To derive parameters for optimal clinical efficacy of photobiomodulation, we recommend a more rational approach, underpinning clinical studies with research of molecular targets and pathways using well-defined biological model systems enabling easy translation of optical parameters from in vitro to in vivo. Furthermore, special attention needs to be paid when conducting studies for hair regrowth, aiming for double-blind, placebo-controlled randomized clinical trials as the gold standard for quantifying hair growth.

We identified key reasons causing an ambiguity around the sensitive skin phenomenon. We propose using standardized selection methods of subjects by a multifactorial questionnaire spanning a range of provocations, including those of chemical, mechanical and environmental origin, followed by clinical, histological and top-notch biophysical measurements. Based on the experimental data obtained from ex-vivo measurements on human scalp hairs and in-vivo measurements on hair and skin, we exemplify that the underestimation of the birefringence content resulting from the orientation related effects associated with the use of linearly polarized light for imaging tissues containing wavy birefringent structures could be minimized by using radially polarized light.

SENSuaL: A physico-biological model of skin irritation following mechanical and light-based challenges

Acute and chronic traum-iterative challenges of the skin are the reality of life. The skin has developed during evolution as the organ to adapt to these challenges and by doing so to protect the ‘milieu interieur’.
Physical challenges can be: (i) acute mechanical challenge (wounding) or chronic mechanical challenges (rubbing, friction, and shaving) and (ii) photo damage, again as an acute or as a chronic form.

The response pattern of the skin is intended to recover the integrity of the skin and to protect against microbial and chemical damage. The initial part of the response is an inflammatory response to eliminate body-foreign elements or altered self (damaged cells) and the secondary response is regeneration of the skin.
The response pattern may follow a normal dynamics with the usual subjective sensations of the skin (a certain degree of pain or itch), with some degree of inflammation (erythema, induration). Some subjects may have aberrant responses with feeling of excessive irritation or signs of irritation (Koebner phenomenon in psoriasis).
Some challenges have become part of our way of living: shaving or laser treatment in skin care procedures (e.h., hair removal using photo-epilation and skin rejuvenation using fractional photo-thermolysis). However, irritation of the skin and the feeling of burning skin remain to be a serious limitation for these common procedures. The latter one imposes restrictions on the efficacy of skin care procedures and on the applicability for all ethnic groups and in particular for subjects with the so-called “sensitive skin” .
This proposal aims to find out the biological reality of “ sensitive skin “ and irritation originating due to the following procedures:
I. mechanical shaving, incl. dry and wet shaving
II. light-based skin treatment, such as:
a. fractional photothermolysis
