To give you an idea of the scale of usage of GLP-1 receptor agonists, in the US, 12% of the population have used a GLP-1 therapy (things like Wegovy or Zepbound). Analysts believe peak sales for GLP-1 drugs could reach $200Bn by 2030. This enormous adoption has utterly transformed the fortunes of Novo Nordisk and Lilly taking them to the top of the market capitalisation tree in the public markets ($874Bn Lilly, share price up 750% in the last 5 years, $400Bn, up 360% in the last 5 years). Oral GLP-1 RA are only likely to increase prescriptions, but with that comes a greater focus on the long term outcomes of patients who have taken these therapies. Known issues around muscle wastage and long-term weight regain will bring greater scrutiny as these drugs become population level interventions. I spoke about pivots in a recent post being driven in DeepTech and BioTech by exceptional science and this announcement from MitoRx Therapeutics is a great example of how experimental data can inform key commercial strategy and how the next wave of obesity therapies can be developed here in the UK. AI Tech in Focus   Brain Scans and AI Used to Detect Dementia Nine Years Before Conventional Diagnosis Researchers at Queen Mary University in London used machine learning to assess cognitive communication between 10 areas of the brain that have been shown to be vulnerable to dementia. The study involving 1,100 people found that using AI analysis of the MRI brain scans of these neural networks accurately predicted about 80% of cases of dementia nine years before a formal diagnosis was actually made. Further, the AI technology was able to predict within two years exactly how long it would take before the onset. The test takes 10 minutes. Dementia is caused by certain proteins in the brain, but patients can live for years with those proteins and show no signs of dementia. Current methods used to detect dementia involve memory tests and measurements of brain shrinkage. It’s hoped that the early detection using the AI trained scans will help identify people who are going to develop dementia and benefit from early treatments. 📌 Founders raising via SAFE—read this! Dan G. does a fantastic job explaining the rationale behind discounts vs valuation caps. So many VCs have blanket responses to a SAFE that is discount-only and doesn’t provide a valuation cap. Understandable at a basic level but does not take into account the differences in types of businesses. My lawyers at Troutman Pepper (Michael Seymour & Jake McDonough) advised on a discount-only SAFE. To be honest, it was frustrating when VCs would say “we don’t invest in discount-only…”—no rationale ever given, just a stance set in stone, I guess. However, Dan does a great job of outlining why a SAFE should be discount only vs valuation cap. For businesses where the future valuation is less predictable (e.g. deeptech—tech solutions based on scientific or engineering challenges) rather than a me-too downstream application (the next best Angry Birds)—a discount is the best approach. It's good to have thoughtful lawyers to provide guidance on making the right early decision on what is best, and thanks to Dan G. for making it clear that there is value (for the company and the investors) in this approach. Other founders will find this article interesting if you are considering which avenue is most appropriate for your business—and that is what is most important. Not creating a structure because VC firms “don’t invest in that one…”-- choose the vehicle that is right for your business and industry. According to the latest report from Rock Health, digital health funding in Q1 2024 totaled $2.7B across 133 deals, with an average deal size of $20.6M. It doesn’t break out device-related investments, but most medtech founders I meet these days are being forced to chase tiny rounds made up of endless little checks, consuming valuable energy and slowing their progress. The benign neglect of this next generation of diagnostic and therapeutic devices is counterproductive and surprising. Given the crowded virtual care market, for example, I’d expect more such companies would be exploring medtech partnerships, investments or acquisitions that'd enable them to back integrate into delivering sharper, and more differentiated, care and treatment. This is exactly what Maverix Medical is doing from the other direction: Reshaping the entire spectrum of lung cancer care by integrating more advanced + effective approaches to screening and diagnosis, treatment and therapeutics, and even palliative care…And putting it all to work through an innovative provider network. One notable investor exception is Y Combinator partner Surbhi Sarna who’s been backing great teams like Selera Medical (YC W24) doing foundational work on ambitious medical devices. Without more support from investors like her, we’re going to increasingly find ourselves stuck with the incremental portfolio improvements from the incumbents, and that’s just not nearly enough.

Is the UK viable for HealthTech Start-ups? Yes and No. Yes - If you are looking to build and validate well. No - If you only have very short timeframes. I speak to quite a few international companies and founders looking to expand within the UK via Adopt-A-Doc. Inevitably I am asked the question above. 𝐋𝐞𝐭'𝐬 𝐜𝐨𝐧𝐬𝐢𝐝𝐞𝐫 𝐭𝐡𝐞 𝐩𝐫𝐨𝐬 𝐚𝐧𝐝 𝐜𝐨𝐧𝐬. ✅ 𝐏𝐫𝐨𝐬: - There are quite a few grants to support early stage companies. - NICE and NHS endorsement carry significant weight globally. - There are plenty of reputable academic institutions to partner with. - MHRA regulatory status, like the FDA, is globally recognised.  - There is a very strong healthtech community within the UK. ❌ 𝐂𝐨𝐧𝐬: - Much higher cost to entry and development.  - Significantly more regulatory barriers and standards to comply. - Timelines are protracted be it in academia, NHS or industry. - The NHS is under tremendous budgetary constraints. - There are many incumbents in place, competition is fierce. 🤔 𝐒𝐭𝐚𝐫𝐭-𝐮𝐩𝐬 𝐚𝐥𝐬𝐨 𝐡𝐚𝐯𝐞 𝐭𝐨 𝐜𝐨𝐧𝐬𝐢𝐝𝐞𝐫: - Are there established procurement frameworks or pots of money for similar technologies? - Are there specific grants or academic institutions that can partner with you? - Does your product currently align with the goals and priorities of the NHS? 🌞 𝐎𝐧𝐞 𝐩𝐨𝐬𝐢𝐭𝐢𝐯𝐞 𝐩𝐨𝐢𝐧𝐭 𝐈 𝐡𝐚𝐯𝐞 𝐭𝐨 𝐦𝐚𝐤𝐞 𝐢𝐬 𝐭𝐡𝐚𝐭: - Compared to many other countries, the UK genuinely has a supportive ecosystem to help healthtech innovators. The Health Innovation Network, NHS Innovation Accelerator, Digital Health Playbook, ABHI (Association of British HealthTech Industries), NIHR (National Institute for Health and Care Research), NICE - National Institute for Health and Care Excellence, Accelerators like Propel@YH and DigitalHealth.London Are all examples of tangible support. (Few countries have all the above) ____________________________________ There is never a right or wrong answer. Each founder have to make the call for themselves. As always, a superb report from Andreessen Horowitz (Big Ideas in Tech 2025) on trends for 2025. The headline "Big Shift in Biopharma" particularly resonates with me, which is why I’m betting on DNA LM.ai, where we’re building a Foundation Model for DNA Below are a couple of trends: 1. Nuclear Energy Renaissance By 2025, there’s expected to be a surge in interest in nuclear energy driven by regulatory reforms, public enthusiasm, increased investments, and growing energy demand, especially for AI data centers. These changes are already leading to the reopening of old nuclear plants, like Three Mile Island, which is set to return to operation in 2028. The growing electricity consumption in the U.S. makes nuclear energy a key pillar for sustainable development and competitiveness in the AI industry. 2. New Frontiers in Future Professions The role of engineers working at the intersection of hardware and software is becoming increasingly critical. The demand for specialists in fields like electrical engineering, mechatronics, robotics, and manufacturing will grow thanks to technological breakthroughs and the rising autonomy of industrial and defense systems. Opportunity to co-design and pilot CLEAR’s AI-powered clinical notes summary tool   The National CLEAR Programme is delighted to be announcing the launch of a seven-month AI pilot which is open to seven NHS trusts across England.   It follows the programme’s development of a new clinically-designed AI powered patient consultation summary tool which can transform workforce productivity enabling more time for patients.   Expressions of interest are now being sought for the pilot which offers seven NHS trusts (one per region) to become early adopters and co-designers of the tool within pre-operative assessment (anaesthetics).    Two webinars providing further information are being held on Wednesday 29 May and Tuesday 4 June 2024. The debate over which is better - 'generalist' or 'specialist' VC is one that will never be resolved. A general partner team skilled at investing, with differentiated deal flow, and a good sense of timing will always rise to the top - whether they specialize or not. However, as described in PitchBook, the data does seem to suggest that specialist VC funds can show favorable results in some markets. From the article: "Healthtech and pharma companies backed by firms with deep expertise were far more likely to record a successful IPO or acquisition than their generalist peers—with exit success rates of 75.5% in healthtech and 82.6% in pharma. Those exits imply better returns for LPs: The median multiple on invested capital for pharma deals led by specialist firms was 1.5x, versus 1.3x for generalists." UK Academic Brilliance → US Commercial Execution: The Reality of Healthcare Innovation I’ve spent the past few weeks in California meeting with a number of people involved in healthcare and amid much debate focused on what the incoming administration may or, may not do, the conversations have brought into sharp focus a pattern I’ve observed time and again in healthcare innovation. While the UK excels at breakthrough research, the path to patient impact increasingly runs through the US market. Here’s why: 1. Stark Value Creation Dynamics: Our academic institutions generate world-class intellectual property (IP) at highly efficient costs compared to US institutions. Yet this IP often flows west, drawn by the deeper pools of risk capital and clearer paths to market adoption available in the US. 2. The Innovation-to-Commercialization Pipeline: From AI diagnostics to surgical robotics to advanced therapeutics, the pattern is strikingly consistent. UK-born innovations gain scale through US healthcare partnerships. Technologies developed in British labs achieve first commercial traction in US hospitals, where procurement moves at market speed. Similarly, breakthrough therapeutic approaches often see their first approved products emerge through US regulatory pathways. 3. Talent Follows Capital: By Series A, UK-based CEOs begin to split their time between the two markets. Post-Series B, commercial leadership gravitates to US hubs, drawn by compensation packages 2-3x higher than in the UK and the scaling experience US ecosystems offer. What starts as a “temporary US presence” frequently evolves into a full headquarters relocation. 4. Market Dynamics Accelerate the Shift: The UK’s single-buyer NHS system demands extensive validation and long lead times, while the US offers multiple entry points for innovation. US investors price in future potential, not just current traction, and their higher risk appetite accelerates development timelines. The Irony of the Loop: This creates a familiar trajectory: UK academic brilliance → US commercial talent → US market validation → global scaling → eventual UK adoption (at premium prices). In effect, UK taxpayers fund breakthrough research, only for these innovations to be bought back after US validation. What Next? This dynamic raises critical questions: Are we comfortable serving as the world’s research engine but losing out on commercial value? Should we rethink how we support the transition from academic brilliance to commercial execution? It’s time to challenge the status quo. How do we create healthcare innovation champions that scale globally while retaining UK foundations? How should HealthTech Founders approach Venture Capital Funding? This was the topic of discussion we had today with our Adopt-A-Doc Clinicians and Founders group. Delivered by the brilliant Estia Ryan from Eka Ventures. ✨ Here are some key points that were made: 🚀 𝐕𝐞𝐧𝐭𝐮𝐫𝐞 𝐂𝐚𝐩𝐢𝐭𝐚𝐥 𝐫𝐞𝐪𝐮𝐢𝐫𝐞𝐬 𝐡𝐲𝐩𝐞𝐫𝐬𝐩𝐞𝐞𝐝 𝐠𝐫𝐨𝐰𝐭𝐡 𝐢𝐧 𝐚 𝐜𝐨𝐧𝐝𝐞𝐧𝐬𝐞𝐝 𝐩𝐞𝐫𝐢𝐨𝐝 𝐨𝐟 𝐭𝐢𝐦𝐞. This requires ambitious #founders with plans of scaling beyond conventional means. Given how funds are set up and the expectations placed upon them, #VC can only make firm investments in companies that can meet those lofty targets (or at least are trying to). But this can be tricky given point number 2, which is... 🛑 𝐓𝐡𝐞 𝐍𝐇𝐒 𝐢𝐬 𝐚 𝐫𝐞𝐚𝐥𝐥𝐲 𝐝𝐢𝐟𝐟𝐢𝐜𝐮𝐥𝐭 𝐩𝐥𝐚𝐜𝐞 𝐭𝐨 𝐬𝐞𝐥𝐥 𝐭𝐨 𝐜𝐮𝐫𝐫𝐞𝐧𝐭𝐥𝐲. The NHS budget is in DIRE straits, and contracts are increasingly difficult to come by with ICBs. #Startups are increasingly having to explore outside of the #NHS - be it in the employer, insurance or private space for avenues of growth. A pure NHS play may be difficult to pitch currently. 🥇 𝐈𝐧 𝐭𝐡𝐢𝐬 𝐢𝐧𝐜𝐫𝐞𝐝𝐢𝐛𝐥𝐲 𝐜𝐨𝐦𝐩𝐞𝐭𝐢𝐭𝐢𝐯𝐞 𝐬𝐩𝐚𝐜𝐞, 𝐕𝐂 𝐟𝐮𝐧𝐝𝐬 𝐚𝐫𝐞 𝐚𝐭𝐭𝐫𝐚𝐜𝐭𝐞𝐝 𝐭𝐨 𝐨𝐮𝐭𝐥𝐢𝐞𝐫𝐬. <1% of all pitches to VC firms gets funding. Founders have to demonstrate traction or #outlier abilities. - They should capitalise on their prior acheivements (be it in sports, previous employment or awards) to demonstrate why they are an 𝘖𝘜𝘛𝘓𝘐𝘌𝘙 founder that can and will go the distance. - Be 𝘋𝘌𝘛𝘈𝘐𝘓𝘌𝘋 about the current traction you are receiving - the health boards, insurance companies, private health systems that are in conversation with you. - If operating in a competitive space, demonstrate clearly how and why your team will be able to 𝘖𝘜𝘛-𝘌𝘟𝘌𝘊𝘜𝘛𝘌 your competitors. 🔬 𝐑𝐞𝐬𝐩𝐨𝐧𝐬𝐢𝐛𝐥𝐞 𝐕𝐂𝐬 𝐚𝐫𝐞 𝐢𝐧𝐜𝐫𝐞𝐚𝐬𝐢𝐧𝐠𝐥𝐲 𝐚𝐰𝐚𝐫𝐞 𝐨𝐟 𝐭𝐡𝐞 𝐧𝐞𝐞𝐝 𝐟𝐨𝐫 𝐫𝐨𝐛𝐮𝐬𝐭 𝐫𝐞𝐠𝐮𝐥𝐚𝐭𝐨𝐫𝐲 𝐚𝐧𝐝 𝐬𝐜𝐢𝐞𝐧𝐭𝐢𝐟𝐢𝐜 𝐞𝐯𝐢𝐝𝐞𝐧𝐜𝐞. Day Zero dispatchs from HLTH Europe For an event that hadn't officially opened yet, there was a huge amount going on yesterday. Adequately fueled and hydrated, your intrepid avatar launched into the festivities. What were some of the highlights? HealthXL hosted an event looking at AI in Lifesciences and Pharma. They always draw a huge crowd on leaders from industry, and today was no different. Chandana Fitzgerald started with the news that they have merged with HLTH, becoming HLTH Community! Robert Garber led with a VC summary of the state of AI, and how to avoid the infamous Trough of Disillusionment. With 82% of companies looking to implement AI governance, it's a good time for those looking to make AI safe and effective. Unsurprisingly, hes also seeing greatest focus on the less complex, administrative side of healthcare. Cristina Ortega Duran gave a lively presentation on their successes with Evinova, which integrates trail data into a single platform. Stephen R. called on the audience to follow 3 rules: focus on the customer problem first, learn quickly, and follow the evidence. Yes, he's turning everyone into Product Managers. During the panel we had great questions about bias and representation from Aahuti Rai, which underlined the role that synthetic data and telehealth have to play in filling gaps and reducing the burden of trial participation. The final workshop focussed on priority use cases for AI in Drug R&D and Clinical Development. Some suggestions included Ambient Documentation for researchers, summarisation of content translation to ease recruitment, and the identification of novel outcome metrics. After hours we had the customary networking events, including a brilliant get together from AlbionVC (thank you Molly Gilmartin!) where I finally got my chance to meet with Hello Self's Charles Wells, reconnect with AI legend and founder of Newton's Tree Haris Shuaib, Kelly Klifa & Alexandra (Sasha) T. from Ally Health, Samantha Fay from SiSU Health UK, and welcome back Dr. James Somauroo & Jessica Somauroo after their 5 week honeymoon. Given how hard they work, the absolutely deserved it. For all the clenched teeth and hair pulling that our industry can cause, today showed that the determination and drive to fix the problems we face are still there.
