What's your Green Coat Index score? That's the question I was pondering after reading Dr Emma Presern 's brilliant and honest article about burnout in the NHS…and also a post from the insightful Dr Ndubuisi 'Andy' Egwim . Back when we applied for medical school, our grades, activities, and personal statements contributed to our WHITE Coat Index, indicating our suitability for clinical medicine. It also reflected our conformity to norms and ability to perform predictably and effectively. Falling short on this index meant rejection from medical school. But what if every Medical School also considered your GREEN Coat Index? Quick definition: The Green Coat Index is a measure of the financial independence, career flexibility and personal fulfillment of current/aspiring Doctors. Not only would you need to have a high enough GCI score to get into Medicine, you would need to maintain a high enough score to STAY in Medicine. Here's what the scores would look like: ### **4.0 (110-130):** A* (Green Star) - Exceptionally prepared. Demonstrates robust financial resources, deep understanding of alternative careers, excellent flexibility, and meticulous planning. Psychological resilience, rich life experience, and meaningful family relationships enrich readiness for medicine and other pursuits. **3.5-3.9 (90-109):** A (Green Ribbon) - Very well prepared. Exhibits solid financial resources and planning, commendable exploration of alternative careers and flexibility. Enhance strengths while nurturing resilience and embracing diverse experiences. **3.0-3.4 (70-89):** B (Green Badge) - Moderately prepared. Possesses some financial resources, planning, and flexibility but may benefit from improvement. Focus on fortifying financial foundation, adaptability, and deeper connections. **2.0-2.9 (50-69):** C (Green Leaf) - Needs improvement. Limited financial resources and flexibility. Prioritize enhancing financial literacy, exploring alternative careers, and accumulating diverse experiences. **Below 2.0 (Below 50):** D (Green Sprout) - Not well prepared. Significant improvement needed in financial resources, planning, and flexibility. Invest time in building a sturdy financial framework, broadening career horizons, and nurturing personal well-being. Following the 'baby steps' advice I gave on Mat Daniel 's podcast, I quickly created a GCI prototype with ChatGPT for Medic SOS readers who clicked the link on the last page of the book. We are all connected with vaccines in some way - whether we are eligible, care for someone or work in research, development and deployment. Preventing ill health and death is, without doubt, the best for individuals, communities and the public purse. The publication from Future Health (below) brings 3 points to mind. 1. Immunisation programmes must be integrated and prioritised in the provision of health, public health and social care. The NHS Vaccination Strategy (2023) sets out goals for a high quality, convenient to access and tailored to the needs of local people and delivered in a joined up way by integrated teams. Integration, ensuring access and improving coverage can be achieved if those working at a local level to deliver programmes are given the resource, capacity and support to do so. 2. Uptake rates for vaccines have been slowing and declining, in adult and child programmes, for some years. (COVER data) These declines may be small % but in the prevention in infectious disease, small can have a big impact. Much was learned during Covid-19 that showed how provision of vaccine services in non-traditional health settings, relevant to local communities, made these vaccines more accessible. Like many, I am concerned that these lessons learned are becoming a distant memory for those in positions to make investment and prioritisation decisions. For example, in South Lincolnshire people had a 10 mile journey to the nearest Covid-19 spring booster clinic in 2024 (not helped by the lack of public transport). The Covid enquiry is due to consider the vaccine programme in 2025 is less than helpful in avoiding loss of learnings. 3. Individual and community responses to the offer of vaccination are influenced by ease of access and hesitancy. There is a big gap in the knowledge and evidence to understand and address hesitancy. Simply put, better understanding of how we behave in this context will support how programmes are structured to improve access for people. It is important that attitudinal and qualitative evidence is collated (researched) with the same degree of scrutiny and importance, alongside scientific, clinical and economic information. I recently attended the "How to tell your Sustainability Story" event hosted by the ever-knowledgable and inspirational Alex Pearson using the Small99 Carbon Reduction Action Box (CRAB). I became certified Carbon Literate a decade ago but it for sure pays to keep learning. Using the CRAB, it was a stark reminder just how much of an impact decisions about who we bank with make. My business name wasn't just because I like the sound of the word. I'm a human being first and business owner second which means I am not perfect. But I strive every day in my work to conduct my business in line with my professional and personal values. I definitely don't always get it right and I may not always make the progress I wish for, but I keep trying. And when it comes to regenerating and protecting the world we live in and operating in ways that are right for people and planet, it's the small actions we take that make all the difference. A couple of weeks ago I facilitated a session on the International Stress Management Association UK panel where Professor Sir Cary Cooper,CBE talked about SME's not doing this properly. I disagreed, yes to some extent and argued that I've seen from my own clients and talks that I've given, quite a few people who do want to do workplace mental health right who have been either Directors of Operations or senior leaders, but have lacked the strategic insights to do so. In an era where mental health awareness is more critical than ever, these standards offer a roadmap for building supportive, resilient workplaces, even in resource-constrained environments typical of many SMEs. Thanks National Voices, UCL Global Business School for Health and UCL Innovation & Enterprise for inviting me and VCSE Nutriri along to talk about how we can start to transform health management services across the country today. We talked a little about measuring the wrong things and the impact of doing so. A bit of ‘future hopes mapping’ and a bit of clarity came for me… that we basically have 2 cohorts in population health management (when it comes to weight at least)… Let’s call them ‘prevention group’ and ‘equity group’… And not prevention of weight change… simply poorer health prevention! Obvs. And the ‘equity group’?… well we’re the group that have felt the most impact of NOT having a weight neutral/inclusive - bias and stigma aware - whole system approach to health at every size… and we’re looking forward to some more stigma informed conversations to come. #MeasureHealthNotWeight #HealthEquityNetwork #NationalVoices Thanks Heather Eggins for the lush company and eggsellent support 😊

🩺 A Root Cause Analysis of NHS Decay: Red and Blue, Both Share the Blame 🩺 NHS's founding principles: healthcare as a right, not a privilege. As a coach working across the NHS, I see firsthand the crude tools that many of our teams are left to work with. From underfunded wards to outdated systems, the challenges are real and persistent. Wes Streeting's comments about the "broken" NHS hit home for many of us, but this isn't a recent issue. The truth is, both red and blue governments have allowed the NHS to decay over decades, creating a fertile ground for privatisation to seem like the only "valid" solution. 🔍 Decades of Underfunding From chronic underfunding to the lack of long-term workforce planning, governments of all stripes have contributed to the NHS's current state. I’ve seen incredible staff trying to do more with less, fighting against a tide of increasing demand and shrinking resources. But let’s be honest—no matter how much passion and commitment NHS workers bring to the table, they need the right tools and support to do their jobs. 🏥 The Push for Privatisation The challenges we face now serve as excuses for privatisation. Rising wait times, understaffed hospitals, and overwhelmed primary care services are spun into narratives that suit a particular agenda. Instead of addressing these root causes, we're told that selling off parts of the NHS is the answer. But is it? 💡 Moving Forward Streeting's acknowledgment of the problems is necessary, but the solutions need to come from rebuilding what’s broken—not selling it off. True reform means involving staff, patients, and communities in the decisions that affect them, ensuring we don’t lose sight of the NHS's founding principles: healthcare as a right, not a privilege. After a disappointing Pathways Funding experience, we wanted to share a post similar to Milly's, but we needed time to digest it. Thank you, Milly Tamati. Looking at the comments, most of us are heading towards a bootstrapping model. This means that the government will miss out on innovative solutions. Last week, the First Minister discussed valuing social enterprise/ entrepreneurs at the Social Enterprise Scotland Summit. However, he failed to address the issue of funding disparities and low morale, which was kicked off by the disappointing way Young Enterprise Scotland was treated. And now that the rest of the Pathways Funds have been distributed, we find that SG has paddled back on funding and working directly with underrepresented founders who created proposals based on research #accessreport and the needs of the communities they serve. Yet most of the funding has been allocated to intermediary organisations that will soon come knocking on the doors of underrepresented organisations and offer scraps or exposure "payment" or "collaboration" to gain access to the communities they don't fully understand or know how to work with. This begs the question: Is it worth being a social entrepreneur in Scotland? For me, these couple of years have been a big No. We will continue to contribute to the economy to support people out of poverty, but we will not continue to try to contribute to government agendas directly if they can't value all of us equally. I can't wait to hear the reasons behind these funding decisions, but for now, we are regrouping and opening up our services to a broader market. We are now launching the same programme we proposed with Pathways to ensure we can provide the modern support ALL entrepreneurs need to navigate these challenges and generate income.

I had the honour of being part of shaping the Birth Trauma Inquiry and policy for how government, the NHS and companies can support parents after birth trauma. The report event on Monday included the Secretary of Health and Theo Clarke MP speaking about the importance of this report and promising that change would be made.    What does this report mean for your company? There are a few key findings that relate directly to corporate companies and how you can support your working parents. If you are not already offering Postpartum Plan to your employees now is the time to speak to us.    1. 'Provide support for fathers and ensure nominated birth partner is continuously informed and updated during labour and post-delivery.'   Postpartum Plan is for ALL parents. 'Provide universal access to specialist maternal mental health services across the UK to end the postcode lottery.'   It should not matter where you live; we should all be able to gain access to good maternal mental health services. It’s been a real honour to work with the Alzheimer's Society to analyse the total cost and prevalence of dementia in the UK. It is particularly meaningful as both my parents have Alzheimer’s and have personally experienced the challenges associated with the disease. A huge burden of the care is provided by carers and families, with our report estimating that total costs will grow from £42b to £90b to 2040 and approximately 63% of the costs of dementia are self-funded. CF report puts a spotlight on the current context of dementia in the UK, and we hope that it informs improvements in diagnostics and new treatments which we all keenly await. It is a novel application of Real World Evidence using patient level data from a newly created Dementia cohort in North West London with Discover NOW and scaled up across the UK. I will be presenting at the upcoming Alzheimer’s Society Annual Conference tomorrow and will discuss CF’s findings in relation to: ·     Projected prevalence and total costs of dementia ·     Total and per person costs by severity and payer ·     New insights about healthcare utilisation I look forward to contributing to these important discussions about prioritising dementia diagnosis in the UK. ✅ I was one of the 1,452 women who applied for Innovate UK Women In Innovation grant. ❌ I was also one of the 1427 ♀️ who didn’t get funded. Part of the feedback from the assessors was that men's hormonal health - in particular Andropause - was a "grey science". Lacking support from the NHS (their source) that it has any "noticeable" negative impacts on men's mental, physical & sexual health. Stating in many cases there is no connection between low testosterone & adverse health 🤯. Clearly ignoring the data provided from May of this year sharing a 5 year study that revealed men with the lowest testosterone levels were more likely to die from all causes. With declining levels across men's lifespan reflecting, and even contributing to, chronic disease and mortality in men. Would that also be the same NHS that also have provided Andropause policies to their work force to bring men's health in line with the same way we support women's? Either way, its just another example of how much work is still to be done in men's hormonal health and lack of awareness and education is hindering men getting support for issues that impact 50% of men over 40.. Once again mirroring how menopause only went mainstream when it was destigmatised and (heavily) invested in - yet had existed in silence like Andropause and other men's hormonal health issues for years due to shame and stigma. As with all the knock backs, it just spurs me on to continue leading for more parity in men's health. There's also increasing scrutiny into the ‘Women in Innovation Awards’. When I entered, it was pledged that 50 female-founded and female-led businesses would receive a £75k grant and package of support, from a funding pot of £4 million. 👎But now only half of this - a total of 25 awards - have been granted. Assuming each grant is £75,000 (a total of £1,875,000). This leaves £2,125,000 unfunded at present without explanation💸 The process was challenging & incredibly time consuming. But like the other women who applied – many of whom I know personally – as females only secure 2% of institutional funding 🗯, I persevered with optimism that this was a scheme that championed us & innovation. 📢 The lack of transparency around the halving of awards has left me and many others wondering - what happened to the process? 📢 A brilliant collective of women has started - #Letsfundmorewomen They are lobbying Innovate UK to: 💰 Commit to fully utilising the remaining funding and awarding the 25 grants 🗣️ Engage in dialogue to address the issues raised and implement necessary reforms 🔎 Acknowledge the shortcomings and review the process  🚀 Launch a redesigned award program for 2025, with more funding committed If like me, you’re one of this year’s unsuccessful applicants, please complete the survey to help 👭>>https://lnkd.in/ecgpfZSx

Where do you get your health and nutrition information? Many of my clients turn to podcasts, with Diary of a CEO and Zoe Nutrition coming up frequently. But did you know that podcasts in the UK are not regulated by Ofcom, the media regulator? This means rules on accuracy and impartiality don’t apply. In other words, it doesn’t matter whether the information shared on these podcasts is true, relevant, or even safe! Outrageous and controversial messages often create more engagement and buzz on social media, giving popular podcasts a big incentive to feature speakers who spread misinformation. These speakers frequently rely on classic “snake oil” tactics: cherry-picking or misrepresenting data, pushing conspiracy theories (“us vs them”), relying on personal anecdotes, or appealing to the “natural is better” fallacy. I highly recommend reading this BBC News article (link in comments). While I doubt it will make a huge dent in the issue, I’m incredibly grateful to those working tirelessly to fight health and nutrition misinformation, like David Robert Grimes and Liz O’Riordan who are featured in the piece (check them on instagram, they are brilliant). Earlier this week, we had a fantastic webinar organised by the True Health Initiative about the era of misinformation we live in. NHS England is making it easier to get lung cancer checks - no GP appointment needed. Just walk in for an X-ray if you’ve got a persistent cough. At first glance, it’s a win. Faster access. Fewer hurdles. Potentially life-saving for one of the deadliest cancers out there. But is this a genuine solution or just a plaster over a gaping wound? Here’s the issue: ⭕ 𝗗𝗶𝗮𝗴𝗻𝗼𝘀𝗶𝘀 ≠ 𝗖𝘂𝗿𝗲 Speeding up checks is great, but what happens after? Are treatment pathways ready to handle the extra cases? ⭕ 𝗪𝗵𝘆 𝘀𝘁𝗼𝗽 𝗮𝘁 𝗫-𝗿𝗮𝘆𝘀? If we’re serious about early detection, why not expand this approach to other cancers too? Or other serious conditions? ⭕ 𝗧𝗵𝗲 𝗡𝗛𝗦 𝗯𝗼𝘁𝘁𝗹𝗲𝗻𝗲𝗰𝗸 Without enough staff or resources are we just shifting the the area of the NHS bottleneck? Or even creating a new one that doesn’t already exist? A bold move, but boldness needs evidence to back it up. Catching lung cancer earlier could save thousands of lives, but success hinges on follow-through: ✔ Adequate staffing ✔ Clear treatment pathways ✔ Sustainable funding and infrastructure What’s your take?
