Addressing health disparities isn’t just about fairness—it’s a strategic imperative. By reducing barriers to care and empowering underserved communities, we create a stronger, more effective NHS that delivers better outcomes for everyone. Matthew Custance The current system for deciding which/ when/ where to build new hospitals in the UK clearly isn’t working, and needs to change. PFI in my view didn’t work either, as it bundled building, FM and other services into complex contracts and the PFI consortia quickly became good at stealing sweeties from tge kiddies letting contracts. Has anyone done a comparison of the end to end process for planning and building a new public hospital in UK vs a range of other developed countries with either social health insurance or Bevan systems? My hypothesis is that in the UK there is a much greater degree of political and treasury involvement, leading to delays, decisions which are sub-optimal at a local level, and excess time and money spent planning projects.

In a comment piece for Health Service Journal, Matthew Swindells takes a thoughtful look at the options and pitfalls when NHS trusts come closer together but without actually merging. One of the attractions of not-quite-merging is the flexibility of the model. A full merger runs very deep: not only are senior posts merged, but over time the underlying operational and management processes come together. That includes core IT systems, which might be totally replaced even if they had several years left in them, at a cost of tens of millions (or even more). A group or collaborative structure allows a lighter-touch approach, with middleware making the data look like a merged system to senior managers, but without having to ‘rip and replace’ the underlying IT. But there is a danger of apples-and-oranges comparisons when data is brought together from different sources. The key is taking the trouble to completely standardise the data into a proper Unified Data Layer (UDL)  before merging it, so that the structure and meaning is fully consistent for all the subsequent analytics and reporting to managers.
