    The new special exhibition at the Mütter Museum and Historical Medical Library of The College of Physicians of Philadelphia – “Trusted Messengers” – marks five years since the onset of the COVID-19 pandemic… The exhibition will bring to life the concept of trusted messengers – the people and institutions that individuals and communities put faith in for healthcare information and decisions. The exhibition opens Saturday, March 15 and features materials used in the development of the COVID vaccine at Penn’s Weissman Lab in Philadelphia, photographs and interviews highlighting the experience of Philadelphia nurses during the pandemic and its impact on their mental health, and innovative materials used in local initiatives to encourage COVID safety and vaccination. ECRI is proud to sponsor opening night for Trusted Messengers in recognition of the brave, brilliant healthcare leaders, frontline staff and scientists who helped pull our nation out of one of its darkest periods.     A Message from ECRI’s Executive Director of Total Systems Safety, Shannon Davila: Every day, patients place their trust in clinicians, but when medical errors occur, that trust is broken — with sometimes devastating consequences. As a nurse, veteran, and breast cancer survivor, I’ve seen firsthand the need to improve safety systems across care settings. No healthcare facility should demand perfection from its workforce, and expect them to operate in faulty systems without the proper safeguards in place. Through systems thinking, just culture, and human factors engineering, we can prevent medical errors before they happen. At ECRI, we analyze our dataset of over 7 million patient safety events to help healthcare leaders stop recurring problems, and design systems to prevent patient harm. These datapoints aren’t just numbers; they represent real people, forever impacted by mistakes that should have never happened.     Concern No. 7 | Diagnostic Error: The Big Three—Cancers, Major Vascular Events, and Infections Diagnostic errors are a major public health issue, leading to over 12 million instances of serious harm annually in the US, with conditions like missed cancers, vascular events, and infections responsible for the majority of these errors.     Concern No. 8 | Persistence of Healthcare-Associated Infections in Long-term Care Facilities Healthcare-associated infections (HAIs) in long-term care facilities (LTCFs) are on the rise, with 1 in 43 nursing home residents contracting an HAI daily, and Pennsylvania LTCFs saw an 18.6% increase in infections from 2022 to 2023. To prevent these infections, LTCFs need expert infection prevention staff and consistent training, despite challenges including high turnover and reliance on temporary workers.     A Message from The Just Culture Company's President, David Marx: As we recognize Patient Safety Week, it’s time to reflect on the healthcare industry’s safety efforts — successes and failures. For too long, healthcare — like many industries — believed that making perfection the workforce standard would prevent harm. But as fallible human beings, this approach is unrealistic and doesn’t break the cycle of adverse events. The good news? Healthcare is becoming less punitive, even when medical errors occur. However, we must shift our focus from responding to harm to preventing it — by improving system design and supporting healthcare professionals through a Just Culture, from the executive level to the front lines. Now, with Just Culture incorporated into ECRI’s Total System Safety approach, we’re driving even more meaningful improvements in patient safety. Systems + culture = outcomes.
