  • Burnout isn’t just about high caseloads or long hours—it’s also about feeling disconnected from your work and its impact. At the SIREN National Research Meeting, our research highlighted how closed-loop referral technology significantly reduces burnout among social care professionals. Why? Because it provides visibility, accountability, and connection. One community health worker shared their experience: "Being able to track the outcome of the referrals I send and follow up with partner organizations has been beneficial in combating feelings of burnout. Knowing and having connections to our partner organizations helps to foster a sense of community and belonging for me." When frontline staff can: ✔️ See referrals completed and clients receiving support, it reinforces their purpose. ✔️ Follow up with partner organizations, it strengthens relationships and collaboration. ✔️ Feel part of a larger network, it fosters a sense of belonging and reduces burnout. Investing in systems that support our frontline staff isn’t just good for them—it leads to better, more sustainable care for everyone. How does your organization ensure staff feel connected and supported? #SIREN2025 #SocialCare #SDOH

Citizens Memorial Hospital is improving patient care by integrating the Unite Us closed-loop referral system into MEDITECH #Expanse Electronic Health Record (#EHR). This seamless connection empowers providers to:  ✅ Access patients' social care history ✅ Build and scale a coordinated care network ✅ Track outcomes and measure impact ✅ Identify service gaps and address unmet needs ✅ Monitor at-risk populations—all within Expanse 🗣️ “With the integration in Expanse, we can more efficiently identify and address social care needs at the point of care. It also provides a more comprehensive view of our community’s overall health, helping us better prioritize initiatives that align with our patients’ needs,” says Michael Calhoun, CEO/Executive Director of Citizens Memorial Hospital/CMH Foundation.
