Peter is a Medicare patient living with coronary artery disease and congestive heart failure. Peter struggled for years to manage his health, until #ValueBasedCare made personalized, proactive support possible. During one of his virtual check-ins, Peter’s care manager noticed a concerning symptom that he hadn’t recognized. Value-based care saves trips to the hospital. A recent study by Optum found that Medicare Advantage patients receiving value-based care were 9% less likely to go to the emergency department or be admitted to a hospital. Patients know Dr. Jason Goldman. He greets each one in the waiting room. He walks them back to the exam room. He calls them personally when their lab results arrive.
