Over the years, Henry Ford Health has launched an exclusive concierge contact line for General Motors employees, tailored offerings when they learned workers were more prone to musculoskeletal injuries, and used algorithms to identify patients who are at-risk—even when the patients themselves didn’t know. Value-based care saves trips to the hospital. A recent study by Optum found that Medicare Advantage patients receiving value-based care were 9% less likely to go to the emergency department or be admitted to a hospital.
