The Oncology Nursing Society (ONS) has released a pivotal white paper titled "The Impact of Climate Change Across the Cancer Control Continuum: Key Considerations for Oncology Nurses." This comprehensive document underscores the profound effects of climate change on cancer care, emphasizing the critical role oncology nurses play in addressing these challenges. As we step into 2025, it's imperative to recognize that environmental health is intrinsically linked to cancer care outcomes. Climate-related emergencies can disrupt cancer care delivery, from prevention and diagnosis to treatment and survivorship. Moreover, environmental exposures contribute to cancer incidence and can exacerbate health disparities, particularly in vulnerable communities. ONS's 2024–2027 Research Priorities highlight the necessity of understanding environmental impacts on cancer care (Rosenzweig et al., 2024). By focusing on the intersection of environmental health and oncology, we can develop strategies to mitigate risks, ensure seamless care delivery during climate-related disruptions, and advocate for policies that promote environmental justice. Advancing environmental health isn't just about combating climate change; it's about improving patient outcomes, reducing disparities, and ensuring equitable access to quality cancer care. As oncology professionals, we must integrate environmental considerations into our practice, research, and advocacy efforts. Let's make 2025 a year where we prioritize environmental health as a cornerstone of cancer care excellence! 😇What’s My Go-To Approach for Coding Any Diagnosis? 7 steps you should not miss! As a CDI from a non-clinical background, I’ve always follow a simple yet effective method for validating and coding inpatient cases. It’s my personal checklist—and it works all the time! 👇 🎯 1. Signs & Symptoms 🗺️ Always start here! What’s the patient complaining about? Abdominal pain, for example, could mean colitis, UTI, or hernia. Patient complaints often point us in the right direction. 🎯 2. Diagnostic Data 🧩 Lab tests and imaging are your allies. Abnormal findings can confirm or rule out suspicions. A radiology report confirming hernia or urine analysis pointing to UTI? Game-changer! 🎯 3. Treatment Clues 🗝️ What’s the doctor prescribing—or not? Treatment plans and contraindications often reveal the core diagnosis. Example: Managing a patient with AKI and acute heart failure differs significantly from treating these conditions separately. 🎯 4. Progress Updates 💪🏿 Don’t skip progress notes! How are symptoms evolving? What do repeat labs or imaging show during the stay? These updates guide you closer to the truth. 🎯 5. The Discharge Summary 👑 This is your best friend. It ties everything together: the diagnosis, treatments, recovery status, and follow-ups. Trust it! 🎯 6. Coding, Facility & Client - specific Guidelines 💎 Want to stand out? Know your guidelines. They’re the secret sauce for nailing the right DRG every time. 🎯 7. Bonus Documents 🤑 Operative notes, nurse notes, and physician query responses during concurrent and retro reviews can provide valuable insights. 📌 Every Case is a Story📃 Patients aren’t just diagnoses—they’re stories of symptoms, tests, treatments, and recoveries. I find a thrill in piecing together the puzzle and presenting it accurately through coding summary. 🩺 Pro Tip: Always trust human doctors and established guidelines. 😉 Google is great for quick references 🧐—but not for diagnoses! 🤭 So what’s your favorite step in coding a case? Central line-associated bloodstream infections, or CLABSI, are almost always preventable if the central lines are assessed and maintained appropriately. These are some of the steps we (nurses) take: 🔹 Assessment: Regularly check the site for signs of leakage, swelling, redness, or infiltration. Assess at least once per shift or every four hours during fluid or medication administration. 🔹 Flushing: To ensure patency, flush the line at least once per shift. Scrub the access port with an alcohol wipe for 15 seconds, then let it dry for at least 5 seconds before flushing it with 10 ml of normal saline. If using a disinfection cap (AKA alcohol-impregnated protective cap or Curos cap), simply discard it, let the site dry, and flush with normal saline. 🔹 Dressing Changes: Change the dressing on the central line access site every seven days or when wet or soiled. Use sterile technique for all central lines except tunneled lines where a clean technique is enough. Gauze dressings are used in case of allergy or oozing but are replaced when soiled or at least every two days. 🔹 In the case of port-a-caths, reaccess every seven days or as needed. 🔹 Tubing Changes: Tubing changes typically align with dressing changes and every 72-96 hours. Keep disinfection caps on unused access ports on tubing at all times. When accessing one for flushing or IV medication, discard the cap and replace it with a new one afterward. If not using the caps, use an alcohol wipe instead. 🔹 Patient Education: Educate to use chlorhexidine soap or wipes at least once every 24 hours during hospitalization, keep tubing off the floor, practice good hand hygiene, and promptly report any issues. If a CLABSI is suspected, prompt testing and treatment are important. Since CLABSI rates are reported at state and national levels, hospitals prioritize prevention. If a patient develops CLABSI early in their hospital stay, determining whether it was acquired before or after admission becomes crucial.
