Zaffer Qasim presented second at #FAST24, leading an eye opening conversation on trauma resuscitation: -We know that <10% of patients with a traumatic arrest, who survive, will have a meaningful neurologic recovery. We need to prevent arrest from occurring in the first place. 20% of trauma deaths are likely preventable. -The goal of trauma resuscitation to stop hemorrhage as soon as possible. It’s that simple, right? Do do this though, we cannot forget the basics. -Hemostatic dressings are still under utilized and are fantastic for junctional wounds and -Managing airways in trauma is particularly challenging because we too often forget what’s going on physiologically including hypotension, metabolic acidosis, hypoxia, and RV failure. Shift your approach from ABC to CCAB: catastrophic hemorrhage control, circulation, airway, and then breathing. -Let’s re-think our focus in traumatic arrest. In medical arrest compressions are the focus because the pump is failing. In traumatic arrest compressions delay fixing the most likely causes: hemorrhage or obstruction. When your trauma patient arrests, focus on restoring volume and eliminating obstructions. -As patients in hemorrhagic shock near arrest, they will vasodilate. This happens for a variety of reasons, one of which is a lack of vasopressin. It may be worth administering it in the peri-arrest trauma patient. -There is a role for calcium in the traumatic patient in hemorrhagic shock, but if you’re going to consider it, you really need to measure your calcium first.
