• Coronary plaque presence increased with LDL-C and clinical risk, even in the absence of CAC. • There is considerable atherosclerosis in statin-naïve participants with low LDL-C and clinical risk. • About 2 in 5 statin-naïve participants with severe hypercholesterolemia did not have any plaque on CCTA imaging. Heterogeneity exists in subclinical atherosclerosis across LDL-C, CAC, and estimated cardiovascular risk levels. The value of CCTA in risk-stratifying asymptomatic adults should be further explored.

During a Women in Nephrology Leadership Conference, Jerri Echeverria, Professor and Provost Emerita, challenged me to rethink what it truly means to honor our word. 👇 Failing to honor our word not only leads others to question our integrity, but more importantly, we begin to question our OWN. There are 6 key principles concerning our WORD: 1️⃣ What you say you would do: Are you fulfilling your promises? And are you doing so on time? 2️⃣ What you know to do: Are you doing the right thing, even when NO ONE is watching? 3️⃣ What is expected of you: (stated or unstated) People have expectations from us. Our kids, our division chief, the promotion committee… Are we meeting these expectations? 4️⃣ What you say is so: Are you making things up? Are your claims based on evidence? As a researcher, this principle is at the heart of what we do. 5️⃣ What you say you stand for: Are you living according to your own values and principles, even when it's challenging? When you say you are committed to excellence, are you REALLY doing things towards excellence? 6️⃣ What you say or do according to moral and legal standards: Do your actions align with both the law and your own ethical compass? ----- In the end, it’s not about how others perceive our integrity, and ALL about how we view it OURSELVES. We are thrilled to welcome Dr. Makoto Hashimoto to our team, further strengthening our hybrid coronary revascularization program. As many of you know, the PREVENT IV trial (Prevention of Autogenous Vein Graft Failure in Coronary Artery Bypass Procedures) reported a 45% saphenous vein graft (SVG) failure rate at 12 to 18 months. Although the hybrid trial could not be completed due to enrollment challenges, most experts agree that the key benefit of CABG lies in the left internal mammary artery (LIMA). At Baptist Health, we’re fortunate to have a surgical team capable of robotic or minimally invasive LIMA to the LAD (via left thoracotomy) while managing additional lesions through PCI. Drug Improves Effectiveness of Radiation for Lung Cancer That Has Spread to the Brain: Researchers have identified a potential new way to make radiation more effective and improve outcomes for patients with lung cancer that has spread to the brain. The study, led by Dr. Debanjan Bhattacharya was published in the journal Cancers. According to the American Cancer Society, lung cancer is the leading cause of cancer death in the United States. Up to 40% of lung cancer patients develop brain metastases and on average survive between eight and 10 months. Bhattacharya and his colleagues at UC focused on AM-101, a synthetic analog, meaning it has a close resemblance to the original compound, in the class of benzodiazepine drugs. AM-101 is a particularly useful drug in the context of brain metastases in NSCLC, Bhattacharya said, as benzodiazepines are known to be able to pass through the blood-brain barrier that protects the brain from potential harmful invaders that can also block some drugs from reaching their target in the brain. The team found AM-101 activated GABA(A) receptors located in the NSCLC cells and lung cancer brain metastatic cells. This activation triggers the “self-eating” process of autophagy where the cell recycles and degrades unwanted cellular parts. Specifically, the study showed that activating GABA(A) receptors increases the expression and clustering of GABARAP and Nix (an autophagy receptor), which boosts the autophagy process in lung cancer cells. This enhanced “self-eating” process of autophagy makes lung cancer cells more sensitive to radiation treatment. Using animal models of lung cancer brain metastases, the team found AM-101 makes radiation treatment more effective and significantly improves survival. Additionally, the drug was found to slow down the growth of the primary NSCLC cells and brain metastases. In addition to making radiation more effective, adding AM-101 to radiation treatments could allow for lower radiation doses, which could reduce side effects and toxicity for patients, Bhattacharya said. Published today, our commentary with Fidel Rubagumya ,M.D, MMed, MPH, PhD (c) in the Lancet Oncology, “Radiotherapy and Conflict: from disruption to expansion and hope” accompanying the Lancet oncology commission on Radiotherpy and Theranostics. We discuss that global disparities in cancer care are most stark in conflict areas and fragile states with significant geographical clustering in Sub-Saharan Africa. Radiotherapy is the most vulnerable cancer treatment modality to disruption and cessation during conflict. Protracted conflicts are a major barrier to developing radiotherapy in the first place such as what we see in Haiti, Somalia, South Sudan and Chad. We discuss factors leading to meagre or absent response to the radiotherapy needs of Africa conflict- affected populations and also in other instances of structural violence and racialized vulnerability in the Caribbean. Actions recommended by the commission can address these disparities but need global solidarity and political support. Medical News in Brief August 9, 2024 WHO Approves First Self-Test for Hepatitis C Article Information JAMA. Published online August 9, 2024. doi:10.1001/jama.2024.14876 In July, the World Health Organization (WHO) prequalified the first hepatitis C virus (HCV) self-test. The product, called OraQuick HCV self-test, is an extension of the OraQuick HCV Rapid Antibody test that the agency prequalified in 2017 for professional use. This self-test kit is designed as a convenient and stigma-free way to reach people who might not otherwise get tested for HCV and therefore go undiagnosed. “Every day 3500 lives are lost to viral hepatitis,” Meg Doherty, MD, PhD, director for WHO’s Department of Global HIV, Hepatitis, and STI Programmes, said in a news release. Of the 50 million people living with hepatitis C, only 36% had been diagnosed and 20% treated in 2022, she noted, adding that the self-test kit would help expand existing services and treatment. The WHO recommended self-testing for HCV in 2021 as part of its goal to eliminate HCV as a public health problem by 2030. Prequalification means the OraQuick HCV self-test will be added to the list of medical products the agency considers safe and effective to distribute in low- and middle-income countries that often lack resources and access to medical treatments.
