🌟 Advancing Stroke Prevention in Underserved Communities: Insights from South Texas 🌟 Proud to share our recently published viewpoint in the European Journal of Preventive Cardiology: “Patent foramen ovale in patients ≥60 with cryptogenic stroke—not a bystander: why diagnosis and closure matter.” While much of my research background has focused on oncology and stem cells, I am always eager to explore impactful topics that address critical healthcare challenges. This work, done in collaboration with the Office of the Health Authority, Health and Human Services, Hidalgo County, Edinburg, TX, examines the role of PFO diagnosis and closure in preventing recurrent strokes among older adults, an often-overlooked but vital interventional topic. This research has a special significance for the Rio Grande Valley’s underserved population, where health disparities can limit access to preventive care. By focusing on stroke prevention, we aim to highlight evidence-based strategies that can improve outcomes for vulnerable communities, particularly older adults at higher risk of recurrence. Beyond advancing cardiology knowledge, this project reflects my broader commitment to leveraging research to bridge gaps in healthcare access, whether through pioneering approaches in oncology, stem cells, or population health. This work is a testament to the dedication of our Hidalgo County health team and our collective mission to address the most pressing health needs in South Texas and beyond. Appreciate the opportunity to speak to our tremendous team. As a preview, here are some of the talking points for my lecture to the group. Part 1. Brief overview of the Field: Computer Vision (Imaging applications & those applications to include {Radiology Radiological Society of North America (RSNA)American College of Radiology, #ComputerAidedDesign} and Digital Health/ #3DPrinting), Speech and Natural Language Processing (Chat GPT and other products in this space), Robotics (with emphasis on medical robotics). Part 2. What this means at the institutional level. I will give examples on how hospitals and universities like us need to - and can - implement a governance plan that ensures to set us up for success. This will include the key aspects of the plan. I will show some of those tough decisions. I will show some of the pitfalls of treating AI “like another add-on for IT”. This time its different. Part 3. What this means at the program level. I will provide examples from our “#DigitalHealth Collective”. This will begin with radiology as computer vision applications to improve the quality of life for our patients, but I expand in scope to describe how we will: create multi-disciplinary research opportunities, get funding, and bring jobs and technology to our region. In an upcoming post, I will send out some teaser slides and some pictures of parts that are 3D printed in the Health Care Facility (HCF) using AI (home grown and commercial) to push the envelope of tech and deliver cost-effective solutions locally. Part 4. What this means at the student level. Our University of Arizona College of Medicine - Phoenix medical students are invited to attend, and this will show how these concepts will make you a better provider, how they will make you more competitive for the match, and how that you can position yourself to participate. We are in the planning of a separate meeting for both the “Radiology” and “AI interest” medical student interest groups at UA and I will be happy to meet with you! Carilion's Neurosurgery Research Eric Marvin, DO, neurosurgeon, is working on research that could reduce side effects and surgical interventions for older adults with chronic subdural hematomas. When older adults experience a head injury—even a relatively minor one—blood can collect between the skull and the surface of the brain, where it can cause symptoms ranging from headache to paralysis or coma. Dr. Marvin’s research could potentially lead to the development of a sustained local drug delivery release system for treating this condition. This new treatment option could help patients avoid additional surgeries after an initial operation. There is also potential to reduce hospital stays and improve patient outcomes. Dr. Marvin in collaboration with third year neurosurgery resident, Kristine Ravina, MD are recipients of our Research Acceleration Program (RAP) award, a grant that funds our employees to start clinical, biomedical or behavioral research projects. Thanks to Diagnostic Imaging Europe & Mélisande Rouger for the opportunity to discuss sustainability during #ECR2024. Key takeaways: 1. Sustainability was already big at #ECR2024. I can't wait for the Planet Radiology theme of #ECR2025 under the visionary leadership of Andrea Rockall. 2. The US has been slower than EU to adopt regulatory and policy approaches towards sustainability. Instead, in the US, promoting sustainable practices rely more on an economic rationale. We can call this approach "Greening the green". 3. Sustainability is more than reducing environmental waste; it also includes supporting a healthcare workforce that is already overstretched, as well as ensuring the financial viability of hospitals, especially in rural areas. 4. Half of the world's population--4 billion people-- are heading to the polls this year. For those who can, let's take the opportunity to vote for leaders who understand the importance of addressing climate change. Our children and grandchildren will thank us!

Very important points about hospital consolidation. ⏺ Has led to the majority of physicians now being employed by a system. ⏺ A hospital system may have different priorities than physicians (in private practice). ⏺A system in which physicians have very little leverage to negotiate or change factors important to patients and clinical care. The real reason highlighted around 2 minutes 50 seconds. Short interview with beneficial insight. ⏺Physicians have joined unions not for wage negotiation but to focus on changes that affect clinical care. We as physicians do not have a unified voice. We have not organized ourselves well to create a unified voice. I am not sure that a union is the right path, but uniting to have a common voice, leverage, and negotiation power is needed.
