Maryland Primary Care Program- the hidden treasure Primary care investment models have searched for the elusive combination of cost savings and quality improvement. The recently released evaluation of the Maryland Primary Care Program (MDPCP) ends that search.  An independent report done by the Hilltop Institute at the University of Baltimore County revealed $161 million in savings, even after the investment of over $510 million dollars to the participating practices to transform Primary Care delivery. Here are a few of the highlights of the evaluation of the first three years. -   MDPCP has over 525 participating practices with 396,000 traditional Medicare beneficiaries (~ half of all Maryland beneficiaries) including FQHCs -   The average reduction in spending (4.3%) per beneficiary of $424/year was calculated based on the Difference in Differences methodology over a matched comparison group -   The investment was made over and above fee for service payments to the providers in the form of Care Management fees and performance incentive payments averaging ~$30 per beneficiary per month -    MDPCP reduced inpatient hospitalization by 7% with the greatest savings in the historically underserved patient groups, adding a measure of health equity -   MDPCP produced additional savings based on improved outcome credits by effectively addressing diabetes and obesity -   MDPCP addressed the substance use disorder crisis through the implementation of a statewide SBIRT reporting dashboard with over 1.3 screenings -   MDPCP saved lives during the Covid pandemic -   80% of the MDPCP practices demonstrated better than the national median for diabetes control quality The credit for this success goes to the hardworking and dedicated primary care providers and the Program Management Office within the Maryland Department of Health  (Executive Director Chad Perman)and the support received from  the state Health Information Exchange (CRISP) providing practice focused dashboards, the data scientists at the Hilltop Institute who created an  avoidable hospital event prediction tool,  the state’s independent Health Services Cost Review Commission, the Maryland Health Care Commission and the guidance of the MDPCP multi-stakeholder Advisory Council.    As many states are making recommendations for enhanced investments in primary care in order to achieve better more equitable health, this model provide a credible path for success. A voluntary state supported program with strong leadership, data systems and transformation support is essential.   One cautionary note- Although the investments made into this program were significant and did produce the desired savings results, it still falls far short of what is needed on a multi=payer basis to build a strong, sustainable primary care infrastructure capable of meeting the growing health needs of our nation Yours in the future of healthcare Howard https://lnkd.in/eFhvskXk

Find out about a new journal article on interventions to promote sleep and rest in the inpatient setting, and read about comments on the topic from Allegheny Health Network CMO Donald Whiting and Chief Experience Officer Eugene Scioscia. "If patients feel rested and feel they are in an environment where staff care about their time and rest, they have a better experience," Whiting says. "A better patient experience helps to determine the patient's outcome. If patients have a better experience, they tend to do better clinically." In the hospital setting, the patient's body is working hard to recover from whatever injury it is experiencing, which can be a surgery or an infection, Scioscia says. "The body is working overtime in terms of its metabolism and healing process," he says, "so it is critical to allow the body to rest." “Nobody’s got more grit than a group of kids with rare diseases” Right now on NBCUniversal, this video is streaming nationally. Montefiore Health System and Albert Einstein College of Medicine have released a short holiday movie that tells a story of a city plunged into darkness, rescued by the ingenuity of a group of kids with rare diseases. What I witnessed as this idea took shape and came to fruition was the intent to portray something realistic among this fantasy — people with rare diseases and their families have to be the resilient ones in society, they work harder to get from A to B every day. This short movie doesn’t pity people with rare diseases, it celebrates them, saluting their grit. At Montefiore Health System, I keep hearing from colleagues in different specialities, adult and paediatric, that everything in the #Bronx is a rare disease. That may be why patients and families say they don’t feel like an unwanted burden on their care providers when they visit us for care. There is a practical component to this outreach — the page below links to our new New York Center for Rare Diseases (NYCRD) website, making it easy for patients, families and referring providers to find rare disease care. Montefiore Health System has over 40 rare disease clinics, with more in planning, a genuinely extraordinary focus for any health system. I’m also gratified that the web content is all available in Spanish as well as English, as rare diseases don’t care what language you speak. While I’ve seen this in development over the last several months, I am still stunned by the results. Working with colleagues like Loreen Babcock and David Bivins and their teams has exposed me to the calm and principled creativity that resulted in this exceptional celebration of rare diseases. The following is MHQP CEO Barbra Rabson's Letter to the Editor published in today's Boston Globe. This was written before today's announcement that ten Mass General Brigham primary care doctors will be switching to Beth Israel Lahey, adding further evidence that our fragile #primarycare system requires urgent attention. Lack of Investment in the Field Sets Off a Downward Spiral in Care We may have finally hit the tipping point for our primary care crisis. In the past week, the Globe published a one-two punch of articles, the first, by Kara Miller, documenting the rise in primary care physicians switching to concierge medicine (“How much would you pay to skip the line at your doctor’s office?” Business, Nov. 18), and the second, by Jonathan Saltzman, covering the drastic move by Mass General Brigham primary care physicians in seeking to unionize because of the “unprecedented volume and stress” they face (“Citing burnout, doctors seek union at MGB”). The same week, at the Massachusetts Health Policy Commission cost trends hearing, Governor Maura Healey called for greater investment in primary care, noting that in this country we spend roughly half of what other developed countries spend on primary care and that our poor health outcomes and health inequities reflect that spending gap. Massachusetts Health Quality Partners and the Center for Health Information and Analysis have published an annual dashboard documenting the failing health of primary care since the COVID-19 pandemic, focused on capacity, finance, performance, and equity. This dashboard, along with the myriad stories published in the Globe and other media outlets, details the depth of the primary care crisis in Massachusetts, which forces residents to seek care in high-cost settings or, worse, forgo care altogether. All of this is driving health care into a downward spiral. It is past time for the governor, Legislature, health systems, health insurers, and employers to take action to make an impactful investment in primary care. Barbra G.
