Health Tech founders often come to me frustrated that they just can’t get hospitals to buy their product. Even though the founder feels that they are solving such an obvious problem for the healthcare system. In my experience, here are the 3 main reasons why hospitals won’t buy your product… and I’ll use shoes as an analogy. 𝟭. 𝗧𝗵𝗲𝘆 𝗱𝗼𝗻’𝘁 𝗯𝗲𝗹𝗶𝗲𝘃𝗲 𝘁𝗵𝗲𝘆 𝗵𝗮𝘃𝗲 𝗮 𝗽𝗿𝗼𝗯𝗹𝗲𝗺 Suppose you looked at my shoes. They’re really worn out. Ripped. Sure it’s annoying when water gets through or my exposed toe gets scratched. But I don’t really care about my feet that much… so my threshold for buying better shoes is really high. Similarly, say you have a solution for recording a patient encounter and summarizing the visit for the patient. While it can be a bit frustrating when verbal instructions my clinicians provided are forgotten and patients need to call us back, it’s not frustrating enough for me to install an entire solution for it. It’s not a true problem for me. You call it a problem… I call it an annoyance. And I don’t spend money to fix annoyances. This is where most Health Tech companies get stuck… most potential customers view you as a Vitamin instead of a Painkiller. You either need to solve a different problem or find an adjacent customer type for who you actually solve a problem for. 𝟮. 𝗧𝗵𝗲𝘆 𝗮𝗴𝗿𝗲𝗲 𝗶𝘁’𝘀 𝗮 𝗽𝗿𝗼𝗯𝗹𝗲𝗺... 𝗯𝘂𝘁 𝗶𝘁’𝘀 𝗻𝗼𝘁 𝗮 𝗽𝗿𝗶𝗼𝗿𝗶𝘁𝘆 Suppose my shoes are so worn out, that I’ve decided it’s a problem I need to solve. I’m convinced I need to buy a new pair of shoes soon. But… it’s more important that I save up for getting my car fixed. That’s a bigger priority for me right now. Similarly, say you have a remote patient monitoring solution to cut readmissions. Sure, my hospital's readmission rate might be bad. I agree with you - it’s a problem that really has to be fixed sometime soon. But that’s a next year priority… because my hospital CEO told me to focus on reducing length of stay this year. So yes, I agree it’s a problem… but it’s not a priority to fix right now. Maybe next year. 𝟯. 𝗜𝘁’𝘀 𝗮 𝗽𝗿𝗼𝗯𝗹𝗲𝗺 𝗮𝗻𝗱 𝗮 𝗽𝗿𝗶𝗼𝗿𝗶𝘁𝘆... 𝗯𝘂𝘁 𝘁𝗵𝗲𝘆 𝗵𝗮𝘃𝗲 𝘀𝗼𝗺𝗲𝘁𝗵𝗶𝗻𝗴 𝘀𝗶𝗺𝗶𝗹𝗮𝗿 𝘁𝗼 𝘁𝗿𝘆 𝗳𝗶𝗿𝘀𝘁 Oh look at that, I’ve got some slippers in my closet. I’ll just wear those. Sure I probably shouldn’t use those to run or play tennis or go out in the rain… but, maybe I can? I mean it’s “free” since it’s already in my closet. Similarly, say you have a patient portal that has more advanced features than what my hospital’s EHR vendor offers. I genuinely believe yours is better. But… I just spent hundreds of millions of dollars on this EHR and patient portal. I know it’s not the same… but maybe it’ll be good enough? Only one way to find out… and yes, that may take months to years to validate. So… let’s stay in touch, but give me some time to try this other option out first. ***** What would you add to the list?

For Health Tech Startups, building a Painkiller product will increase your probability of success… BUT it may also just reinforce the problematic Status Quo👇 Here’s another way of framing it: → Starting with a visionary product in mind often leads to a Nice-to-Have Vitamin (i.e. it’s really hard to get anyone to care about a better future when they’re struggling just to deliver healthcare in the current system) → BUT solving a problem for the current Market sometimes often means you’ll just be making things incrementally better… for an existing suboptimal state of care. Let me explain with an example… When we started SeamlessMD 12 years ago to digitize patient journeys (e.g. pre/post-surgery), it was incredibly visionary because we were asking providers to join us in transforming MANY things at once: → Transitioning from static, one-size fits all paper instructions to automated, personalized digital reminders and  education on mobile/web → Empowering patients to track symptoms and health status digitally at-home (when patients weren’t even being asked to track symptoms on paper) → Providing alerts and dashboards to clinicians to monitor patients at-home and catch problems earlier (when clinicians were at-best doing one post-discharge phone call, but often nothing at all). → Caring to improve clinical outcomes such as length of stay, readmissions, etc. when often there was a lack of financial incentive to do so So unsurprisingly it was a MASSIVE struggle to get health system partners for many years. Lots of enthusiasm and lots of successful clinical studies… but mediocre health system adoption. Luckily we stuck it out and the market factors changed enough over the recent years to evolve our “Vitamin” closer to a “Painkiller”. But if I went back to 2013 with the goal of building a similar business with the highest chance of succeeding, I would start with the Market and work backwards to the Product Vision. I might think to myself: → There is an existing Market category for print education and PDFs → There is a more recent Market category for patient education videos → There is an existing sales and marketing motion for these categories, so if I make a better version of these, I can build a sustainable business And I’m sure we could’ve built a reasonable business out of that. We would probably have spent less time worrying about whether we’d be around in a year and more time actually working with health systems. BUT ultimately we also would not have had the same dramatic impact on helping health systems reduce length of stay, readmissions, mortality and costs in a truly transformative way. That’s why there’s no single right answer to whether innovators should build a Vitamin (that has a small chance of being a future Painkiller) or something that is probably a Painkiller today. Probably the right answer is whatever gives you energy and motivates you to build.

Build your AI or use existing ones for health tech? As a founder, I frequently ask myself this question. We have incorporated 8 different AI models to get closer to our goal: give every human on Earth their personal AI trainer. So, when I need to decide on the AI model, I try to think of it from different perspectives: - Is it something that we, as a team, are experts it?  - Do we have enough resources to build this?  - Do we want to spend the time required to map it out, train, test, iterate?  - Can this wait until later? If you responded “no” to at least 2 of these questions, then you probably should start looking out there. Cuz in a lot of cases, it’s cheaper, faster, and more efficient to just buy the models or use existing solutions. For example, several clients of KinesteX AI Personal Trainer in your Digital Health Platform came to us with the problem that they spent more than $500k, a couple of years, and tons of Human Resources to build what we have and eventually just decided to buy our solution. The world moves quickly and so should we as founders. Your first customers are not investors. Don't pitch them your company. I see this too often. Early stage startups about to approach their first customers use their investor decks as their starting point. Resist the urge to reuse slides and avoid this trap. Your customers mostly care about: ▪️The problem you are solving for them ▪️How your solution is better than existing alternatives ▪️What benefits they will see ▪️Why they should trust you To be successful with your sales efforts, make it about *them*

The shift in Series A to Series B graduation rates is a stark reminder of how the startup landscape has evolved, particularly post-2020. The "sugar rush" analogy fits well—easy access to capital led to rapid growth and inflated valuations. But as interest rates have gone up and funding has become more selective, startups are now having to return to solid business fundamentals. For MedTech and FemHealth startups, the journey from Series A to Series B was never easy, even in more favorable times. Now, with the additional pressure of financial conservatism, these companies must show not only potential but tangible progress in product-market fit, regulatory paths, and revenue generation. This is where smart money, strategic partnerships, and a clear focus on health outcomes can differentiate those startups that are essential from those just chasing the next round. With venture capital taking a closer look at impact and sustainability, founders should focus on aligning with investors who understand the long-term value of innovation in healthcare. For founders currently raising, it's about knowing that this is a marathon, not a sprint. Yes, the competition is tough, but it also means there's an opportunity for those who stay nimble, pivot intelligently, and keep an eye on profitability and growth potential without sacrificing the mission. Is that really a death sentence for smaller startups? I don't think so. Alright, let's take a step back and look at this DTC healthcare situation from a different angle… Sure, the market is tough right now, and big players like Hims are trading at lower multiples than we'd like. But here's the thing: tough times are GOOD. Tough times are when the real innovation happens. It sucks in the moment, but it forces you to get creative and build something sustainable. The same goes for DTC healthcare startups. Instead of relying on endless rounds of VC funding to fuel growth at any cost, these companies now have to focus on the fundamentals — just like every other medical practice and healthcare system out there. All things must eventually survive without huge churn. They have to figure out how to; acquire new customers efficiently, retain them for the long haul, and actually turn a profit. 👆 Crazy concept, right? Some startups might not make it through this rough patch. I’m not one to sugarcoat that. But the ones that do will come out stronger and better equipped to tackle the real challenges in healthcare. With proven business models, loyal customers, and the grit to keep pushing forward even when the going gets tough. Before Side Health I was aggregating, building, and running ENT practices. There was no VC money coming to save the day. Build a proper business plan or die. The same principles apply to DTC health tech. If the current market conditions lead to more startups actually delivering on the promise to make healthcare better for patients while reducing costs, that’s fantastic. So to all my friends out there grinding away at DTC healthcare startups, don't lose hope. Keep putting one foot in front of the other, keep listening to your customers, and keep believing in the power of innovation. The world needs what you're building, even if the valuations don't always reflect it. And to the VCs who are suddenly getting cold feet about DTC healthcare, I have just one thing to say: you're missing out. The startups that emerge from this crucible will be the ones that reshape the industry and deliver outsized returns.

Let's dive in!👇 Valuing early-stage startups🚀 can be challenging due to limited financial data📊 and uncertainty. Various methods are used: 1. Founder/Idea Value💡 Founders' experience and ability to execute, along with the idea's potential, shape the startup's valuation. Key considerations include founder background, market size (TAM), competition, and unique tech or IP💻. This approach is largely qualitative. 2. Comparable Transactions/Valuations🔍 Valuations of similar startups in the same sector can be compared, considering differences in team, product, market, and traction📈. 3. Scorecard Value📝 The scorecard technique rates elements such as team, product, market size, and competitive edge. Predefined valuation ranges inform the startup's worth. 4. Venture Capital Method💰 VCs value startups based on ROI expectations. Exit value and potential ROI are evaluated, assessing market size and targeted return multiple (10x or 20x)💹. Despite using TAM🌍 to estimate market size, it doesn't reveal market share, competition, or execution risks, making it unreliable. Thus, startup valuation combines art🎨 and science🔬, involving both qualitative and quantitative measures. Discounted cash flow (DCF) and revenue multiples gain relevance as the startup generates more data. Founders must understand valuation methods and articulate their startup's worth convincingly, emphasising market potential and team strength💪.

Our client needed an EHR solution. Why we turned to Healthie. The task was to integrate telemedicine into an app and make it work for reproductive health. We spent hours researching EHRs. Healthie turned out to be very flexible for both backend and frontend. Setup was also easy, thanks to user-friendly configurations. Support from Open Loop was great; they helped with integration and financial advice. But we hit some bumps (who doesn’t?): Tight security settings slowed us down; Documentation gaps meant trial and error. Even with these challenges, the sandbox environment helped us test quickly. In the end, Healthie made scheduling and patient data easier to manage, & improved telemedicine for Zuri app. It was a valuable experience for Fulcrum. As someone who’s sold SaaS to the US Healthcare industry during its transition to Value-Based Care, I’m seeing a familiar story unfolding with AI for SaaS. Remember when US Healthcare shifted from “Fee-for-Service” to “Fee-for-Value”? It was messy. Established players resisted because the old model's misaligned incentives were predictable and highly profitable—even if it wasn’t working for the system as a whole. Guess what? SaaS is at a similar crossroad. With Outcome-as-a-Service models gaining traction (thanks to AI), subscription stacking is becoming harder to justify. Buyers are demanding results—not just access—and the pressure is building to prove real ROI. Here’s what SaaS can learn from healthcare’s transition to value-based pricing: 1️⃣ Co-Creation Is Key: In healthcare, patients, providers, and payers had to collaborate to drive better outcomes. For SaaS, that means involving customers in co-creating AI features and value metrics. Deep feedback loops aren’t optional—they’re essential. Customers want to shape what they pay for and see the value clearly. 2️⃣ Post-Sales is the New Battleground: Healthcare’s success in value-based care relied on proactive care coordination and ongoing patient engagement. SaaS needs a similar shift. Customer success, onboarding, and post-sales service will be critical to ensuring customers see continuous value—and stick around. 3️⃣ Data is the Foundation: Healthcare had to adopt advanced analytics and data integration platforms to track outcomes. #CASE_STUDY: AI-Powered Lead Generation Solutions for Ketamine Therapy Clinics by #Harison Mbiseh This case study is one of the best use cases of automation in AI and LLMs right now. With AI assistants becoming more common, it’s not too far off when we’ll all be talking to them to get things done instead of searching online or clicking through lots of web pages. AI assistants probably won’t replace websites and search engines entirely, but they’re becoming a big part of how we use the internet. This case study is about how I created an AI assistant for a Ketamine Therapy clinic. It helps visitors to the website find what they need, get answers to their questions, figure out if Ketamine treatment might be right for them, schedule a free appointment, and leave their contact info. This makes the website easier to use for visitors and brings in more leads and conversions for the clinic. #Problem Statement & Objective Description of the core problem(s) we are addressing Website visitors struggle to find the right information, particularly when their intent and needs vary. Whether they’re looking for treatment, want to collaborate, or need to refer someone, it may be confusing and time-consuming for them to navigate the site and find what they need. Plus, people interested in treatment don’t have the self-assessment tools to know if the treatment is suitable for them or if they should schedule a free consultation. Notes that understanding the core problem and pain point faced by an organization and its audience is very important in defining the best AI use case. I will continue with part two in the next post, where I will share the objectives, the solution, and the tools I used to build and deployed the solution.
