From 2016 to 2023, Epic increased its acute care hospital market share from 26% to 39%; meanwhile, Oracle’s share held flat at about 25% until decreasing to 23% in 2023. Additionally, Epic controls 52% of the acute hospital care market by bed count, more than double that of Oracle. Thus far, Oracle and other, smaller competitors have been unable to curb Epic’s rise, as Epic was the only EHR vendor to increase its hospital-bed market share last year.

How to blow $230 million/year: - Sell a big market forecast with Standard Model thinking - Confuse said big market forecast (i.e., metrics) for a strategy - Point resources and capabilities in the wrong direction - Change said wrong direction early and as often as possible - Assume expert knowledge of the past is the way to construct a future - Ignore, misread or pretend-away the 'ground truth' from the frontline And when the mathematical gymnastics packaged in the PowerPoint don't manifest NOW in terms of the 'next $1 billion in growth' or, even better, delivering on the true center of the American Way of Healthcare (i.e., more shareholder value), throw in the towel, announce layoffs and revert to the place you're comfortable with conceptually. Says Walmart in a statement to Endpoints News: "What we learned through Walmart Health centers will help us continue to innovate and support the healthcare needs of our communities through our legacy pharmacy and vision." Possible, but not probable. By definition, the only place a "legacy" vision can lead is back to the past. If there's a "learning" here -- the kind of thing Harvard Business School can package in a PowerPoint to sell as a new corporate/executive education program -- is that making big market innovation come to life is hard work. It isn't a solo sport (Walmart can/would have fared better collaborating with a big pharmaceutical company on the roadmap). And it doesn't start with a forecast that everyone knows is bullshit, particularly the people buying and selling it. The upside of a new market is unknowable at the start, just ask the team at Amazon/Amazon Web Services (AWS), who ten years ago invented what is now a $600 billion market for 'cloud infrastructure' services. They had no idea it could be that large. But maybe the biggest learning of all is that making a Big Dent happen, particularly in healthcare, takes time, a more original vision and a completely different starting point. There continues to be challenges on the battle fronts of community-based pharmacies. On the heels of the DIR cliff and now newly applied PBM true ups from claims three years ago, a "new program" that continues the opaqueness, convoluted, and complicated system of health plans and their pharmacy benefits has been initiated. This integrated pharmacy benefit with a national drug discount card company and its' additional fees may be the final death knell for many community-based pharmacies leading to more closures and pharmacy deserts. https://lnkd.in/gnRj3pXq

Curious what you think -- do brand insulin manufacturers have a RESPONSIBLITY to continue to manufacture insulin? This is a quote from a Stat News article that is focused on insulin shortages that have forced some countries to prioritize insulin supply - “Of course they’re more interested in GLP-1s now. Of course they are, and they want to maximize their profits before their patents run out,” said Helen Bygrave, a chronic disease adviser for Doctors Without Borders who has been monitoring the shortage of insulin pens in South Africa. “But at the same time, they have a responsibility to make sure people who need insulin continue to get it.” Over the past few years there has been a lot of focus on insulin prices and how they are too high. But if they are too low, they might still make a profit but the company might lose out on the opportunity cost of other areas of focus (GLP-1s, etc.). Biosimilars could step up so it isn't necessarily on the initial manufacturers to produce the product. I guess I see both sides -- moral obligation to make sure patients have access but financial obligation to stakeholders and I suppose moral obligation to patients taking (or wanting to take) GLP-1s. What do you think?

Last week's exciting HLTH USA in Las Vegas was quite the show. All the big names in U.S. healthcare presented, exhibited and hobnobbed with other like-minded pioneers, followers and wannabes with a desire to take the pulse of the current healthcare environment and gain some insight into 2025. Too much to cover in one LinkedIn post, but here are some of the highlights (at least, through my prism): - AI for almost everything healthcare. Google Vertex, AmazonOne, Nvidia, Microsoft Nuance. Even Salesforce has joined the party! - Lots of collaborations and J-V's including Kaiser's Risant, BS of California, Highmark Health, Deloitte, Stanford Medicine, Epic and others. Many established players are putting their heads together while newer start-ups that have jumped ahead are also being rewarded. - Interestingly, Microsoft provided a somewhat tempered view of the AI frenzy by declaring that while AI is the future, the healthcare applications are not quite ready for primetime. - To no one's surprise, GLP-1s were big in the show. Plan sponsors discussing strategy, providers describing their solutions, and compounding/telehealth pharmacies sharing how and why their products can be trusted. - Investors were also participating. In general, they seemed to be very bullish on 2025 for both healthcare start-ups and in their support for portfolio companies. - And then there was the upcoming election! No big revelations or tips. Mostly just walk the issues (ACA, IRA, CMS, Medicare, etc.) that could be affected down the middle. Basically, things may change whoever gets elected! #HLTH #healthcare2025 #AIhealthcare

Fewer than half of patients in the U.S. consider their healthcare provider their top source for health information, according to a YouGov and Healthline Media poll. At a time when patient trust is on the decline, this is a worrying trend, especially with so much misinformation out there.
