Primary care is in crisis in Massachusetts, and we can all do more to support primary care teams. The Boston Globe’s recent editorial highlights the burdens facing primary care in the Bay State, and it is no secret that caring for complex patients is among the most challenging work for primary care teams. Senscio helps to relieve the burden on PCP by supporting complex Medicare patients. Patients referred to Senscio’s Ibis Health Program get coaching on self-management of their chronic conditions, increasing adherence to their PCP's care plan. For those of you who have been following our work on Pathways to Population Health Equity, one beautiful outcome of applying that framework in Rhode Island has been the recognition of the full scope of the #CHW role in building bridges between clinical and community environments to create upstream investments in population and community health. Drawn from communities closest to the challenges, these natural civic connectors, who skilled at relationships and navigation but can also be trustworthy and effective leaders of clinical-community coalitions and partnerships to address upstream community conditions like housing availability and tenant rights and root causes like racial inequities in the environment. In this week's issue of NEJM Group, we share how this led to a roadmapping process among 140 CHWs and allies about how to sustain the role with braided and blended funding and describe the process. The four roadmaps developed to sustain the role of CHWs were about: 1) Growing the role, agency, and leadership of community health workers 2) Workforce development for CHWs and their supervisors 3) Payment and sustainability 4) People and place-based coordinating infrastructure The full roadmaps can be found here: https://lnkd.in/eND39AEU A year after implementation, there remained strong engagement and support for the implementation within the state and advancement of key functions to support CHWs. At SameSky Health and GroundGame Health, we've learned that healthcare is not always equitable because of unaddressed health-related social needs that serve as barriers to achieving positive health outcomes, and we work tirelessly with our health plan and community-based partners to remove these barriers for members. But addressing SDOH needs can take many forms, and the city of Boston is taking some important steps to through its new #healthequity initiative. The groundbreaking $10 million initiative, supported by the Atrius Health Equity Foundation in partnership with the Boston Community Health Collaborative, aims to improve life expectancy and financial resilience in neighborhoods with poor outcomes across three leading causes of premature mortality. Organizations will collaborate to help at-risk residents meet basic needs, access financial support, and navigate complex healthcare and social support systems. This holistic approach addresses critical social determinants of health, including healthcare access, nutritious food, safe housing, and educational opportunities. It's wild to think in 2024 we're FIRST seeing the DOJ accuse telemedicine conglomerates of criminal drug distribution. How long have we known about these practices in brick and mortar care...? Answer: decades! Take it from someone who prescribed stimulants and other psychotropics to kids over the years - painstakingly measuring and documenting the need and impact - these appalling abuses by Done and Cerebral mean we need more calls for thoughtful pause when it comes to working with startups. Particularly about funding's role - my favorite topic! 🤑 How did we end up with venture capital and PE financing in healthcare that risk the safety of kids in the name of exorbitant returns? Not that all VC or PE dollars are bad, of course, but they're certainly key drivers who shouldn't be in that seat. Did we underestimate these funders as 'barbarians at the gate', much like the pharma reps of the 90's and early 2000's heyday? Have we not learned from Purdue Pharma's aggressive and deadly Oxoycontin marketing? Purdue pleaded guilty to criminal charges and paid $600 million in fines in 2007. Or TAP Pharmaceutical Products's Lupron kickback scheme for the prostate cancer drug? TAP also pleaded guilty to criminal charges and paid $875 million in fines in 2001. Or Pfizer marketing Neurontin, an anti-seizure drug, for off-label uses, such as treating pain and bipolar disorder, even though the drug was not approved for these purposes, and paying physicians kickbacks. In 2004, Pfizer pleaded guilty to criminal charges and paid $430 million in fines. Cerebral and Done's playbooks were practically the same thing, just with a modern mid-pandemic, low-regulatory scrutiny, digital-platform twist. Will appropriate regulation ever outpace American venture greed? Is it even possible to ensure patient safety AHEAD of tragedies driven by the intersection of innovation and venture funding? Yesterday I had the opportunity to moderate a panel focused on healthcare cost drivers in Massachusetts. The audience was comprised of a large number of employers. My panel was comprised of subject matter experts who know and understand the data around healthcare costs and drivers. The conversation was robust. Following the presentation as I was hurrying out the door to drive home a member of the audience commented: “That was really great, I didn’t know any of the things you talked about.” Honestly, the comment didn’t surprise me. As part of my introductory comments I polled this audience of healthcare professionals on how many had heard about or knew about CAA 2021 or what it required. Fewer than 1/3 of the audience responded affirmatively. We have a lot of work to do. Suddenly, employers are being asked to be much more involved in their health plans. They’re being asked to understand where money is being spent and to account for it. Few know where to begin. Last week the Rand Hospital Transparency Report 5.0 was released. It is a treasure trove of information for employers. It looks at the huge degree of cost and quality variation between and among health systems in specific regions. It is a terrific starting point for new health plan fiduciaries. With the added benefit of the Sage Transparency tool, employers can quickly and easily get a more granular sense what they’re paying, the appropriateness of those payments and the relative quality of the systems they’re sending members to. Things are suddenly changing in healthcare. Suddenly employers are interested in direct contracting with providers and health systems willing to take risk on their employer populations. Employers are getting the data on what they’ve paid in the past and are discovering that they’ve dramatically overpaid for inconsistent outcomes and patient experience. We’re seeing an actual marketplace of solutions emerge.
