The PCMH model represents a framework of partnerships among clinicians, patients, and their families that puts patients’ needs above all else while striving to engage patients in their own care.4 The collaborative efforts of various clinicians are coordinated by a strategically centralized health care practitioner, most often a primary care physician. In the PCMH model, quality of care and patient safety are paramount considerations, with data captured and used for performance evaluation and provider accountability. All of the principles integral to the PCMH model align with those that served as the foundation for many of the components of the ACA, including the centerpiece of the ACA: accountable care organizations (ACOs).

First introduced conceptually decades before the passage of the Patient Protection and Affordable Care Act, the patient-centered medical home (PCMH) has evolved as a foundational element within the larger health care system or medical neighborhood, highlighting a coordinated and comprehensive disease management approach centered on intensive primary care interventions. More recently, in the wake of health care reform, accountable care organizations (ACOs) have been established to help health plans, physicians, hospitals, home health care practitioners, and other health care providers better coordinate care through an incentive-based payment arrangement. Osteopathic medicine is poised to proactively capitalize on these emerging health care models, with the anticipated end result of improved quality of care and reduced health care costs. As such, osteopathic physicians involved in the prevention and care of patients with type 2 diabetes mellitus need to identify models, best practices, and solutions to advance the medical neighborhood.

The evidence for the PCMH described here underscores the impressive and growing trends that tie
the medical home model of care with reductions in health care costs and unnecessary utilization of
services; improvements in population health and preventive services; increased access to primary
care; and growing satisfaction among patients and clinicians. This is positive news for stakeholders of the PCMH and primary care and runs counter to one widely publicized study of an early PCMH pilot,1 which found no cost or utilization reductions.Other authors -

onset and prevention of smoking, alcohol, and drug abuse
among 526 students from two junior-high-schools in California.
Over two school years, students who were trained to
resist social pressures toward tobacco, alcohol, and drug use
began smoking at less than one-half the rate of those who did
not receive special training. Frequent alcohol and marijuana
use was also less prevalent among the students who received
