A message from ACPA's Government Affairs Committee Chair, Dr. Ritu Prasad, MD, The Centers for Medicare and Medicaid Services (CMS) and United States Department of Health and Human Services (HHS) have requested 𝐜𝐨𝐦𝐦𝐞𝐧𝐭𝐬 related to 𝐌𝐞𝐝𝐢𝐜𝐚𝐫𝐞 𝐀𝐝𝐯𝐚𝐧𝐭𝐚𝐠𝐞 (𝐌𝐀) 𝐩𝐥𝐚𝐧 𝐝𝐚𝐭𝐚 𝐜𝐨𝐥𝐥𝐞𝐜𝐭𝐢𝐨𝐧 𝐚𝐧𝐝 𝐩𝐫𝐚𝐜𝐭𝐢𝐜𝐞𝐬 𝐝𝐮𝐞 𝐛𝐲 𝐭𝐨𝐝𝐚𝐲, 𝟓/𝟐𝟗/𝟐𝟒.   This request comes particularly timely given the recent final rule 4201-F impacting MA and year-after-year increasing enrollment in MA. The American College of Physician Advisors’ Government Affairs Committee seeks to offer CMS comments to help inform CMS and HHS of our experience with current MA practices, concerns regarding data collection, and to ensure that MA plans provide services to Medicare beneficiaries that are at least as generous as those provided to Medicare fee-for-service (FFS) in a transparent fashion and with decreased administrative burden. 𝐖𝐡𝐲 𝐬𝐡𝐨𝐮𝐥𝐝 𝐲𝐨𝐮 𝐜𝐚𝐫𝐞? ・ More than 50% of Medicare-eligible beneficiaries are enrolled in MA plans and this enrollment is anticipated to continue to rapidly increase. ・ There are ongoing concerns that MA plans continue to offer benefits that are more restrictive than Medicare FFS (via not complying with the Two-Midnight Rule, clinical validation denials, using opaque proprietary restrictive criteria, re-defining coverage benefits as “payment disputes,” laborious prior authorization, etc.). ・ Underpayment occurs frequently for services provided, with significant delays in payment to hospitals. ・ Administrative burden to fight inappropriate denials is at an all-time high. ・ Improved transparency and data reporting from MA to the public is imperative. 𝐀 𝐟𝐞𝐰 𝐡𝐢𝐠𝐡𝐥𝐢𝐠𝐡𝐭𝐬 𝐨𝐟 𝐨𝐮𝐫 𝐜𝐨𝐦𝐦𝐞𝐧𝐭𝐚𝐫𝐲 𝐢𝐧𝐜𝐥𝐮𝐝𝐞: ・ Improved accountability for plans with regards to 4201-F, the Two-Midnight Rule and use of internal coverage criteria. ・ Enhanced transparency and timeliness of prior authorization processes. ・ Proposed guardrails around skyrocketing Clinical Validation Denials by MA plans. ・ Request for a clear centralized process to submit comments to CMS regarding MA plan behavior that is publicly accessible. The Oncology Nursing Society (ONS) has released a pivotal white paper titled "The Impact of Climate Change Across the Cancer Control Continuum: Key Considerations for Oncology Nurses." This comprehensive document underscores the profound effects of climate change on cancer care, emphasizing the critical role oncology nurses play in addressing these challenges. As we step into 2025, it's imperative to recognize that environmental health is intrinsically linked to cancer care outcomes. Climate-related emergencies can disrupt cancer care delivery, from prevention and diagnosis to treatment and survivorship. Moreover, environmental exposures contribute to cancer incidence and can exacerbate health disparities, particularly in vulnerable communities. ONS's 2024–2027 Research Priorities highlight the necessity of understanding environmental impacts on cancer care (Rosenzweig et al., 2024). By focusing on the intersection of environmental health and oncology, we can develop strategies to mitigate risks, ensure seamless care delivery during climate-related disruptions, and advocate for policies that promote environmental justice. Advancing environmental health isn't just about combating climate change; it's about improving patient outcomes, reducing disparities, and ensuring equitable access to quality cancer care. As oncology professionals, we must integrate environmental considerations into our practice, research, and advocacy efforts. Elevating Chronic Care Management through Nurse Care Managers As healthcare continues to pivot towards value-based care models, the role of #NurseCareManagers (#NCMs) in Chronic Care Management (#CCM) has never been more pivotal. NCMs are at the forefront of identifying eligible patients, developing personalized #careplans, and ensuring seamless #carecoordination. But how does this translate into better Quality Payment Program (#QPP) scores and increased incentive payments? First, by identifying patients eligible for CCM, NCMs ensure that those with multiple #chronicconditions receive continuous, comprehensive care. They not only enroll patients but also provide them with the necessary education about the benefits of such programs, leading to higher patient engagement and satisfaction. Secondly, care coordination is the heart of CCM. NCMs work tirelessly to connect the dots between various healthcare providers, ensuring that the patient's care plan is executed flawlessly. This includes managing medication, scheduling follow-ups, and monitoring the patient's health status. The seamless execution of these plans leads to improved patient outcomes and reduced hospital readmissions, which are critical metrics in determining QPP scores. Lastly, meticulous documentation and correct coding of CCM services by NCMs ensure that providers are reimbursed accurately for the care provided. This not only supports the financial health of the practice but also reflects the quality of care in QPP scores.
