“So... Apple is entering the hearing space!” For the past two weeks, I've been flooded with messages asking how Apple's latest announcement will impact Concha Labs. A whirlwind of activity last week at the America's Physician Groups conference. Lots of solid insights shared by health system leaders. Some notable thoughts: +++ Artificial intelligence is intelligent in different ways: By now, we all know ai has a diverse set of use cases, incorporated in different applications and tools. But there are fundamental differences in how the intelligence itself is brought to life. Examples include: - Predictive AI, used to forecast future outcomes by leveraging historical data. Shout out to UCLA’s CKD/ESRD early intervention protocol (Joycee Berin) and Diagnostic Robotics’ intelligence care post-dicharge journeys (Kira Radinsky) - Protocolized AI, used to automating a user’s administrative load by using a set of detailed, pre-defined rule sets. Thanks to ARC for sharing their experience using Elaborate 's automated outpatient lab summaries (Manish Naik) - Generative AI, used to create new content. Great overview of ambient documentation at MGB as well as Cedars’ Sinai talking through its medical ontology model powering acute triage (Caroline Goldzweig) Across system leaders and experts, there was clear agreement that the first step to implementing intelligence intelligently is defining the ‘jobs to be done’. That way, internal and external stakeholders are clear on what they're solving for when they architect the solution.  +++ Scaling AI takes time and rigorous observation: Market leaders describe a phased pilot approach that allows clinicians to build confidence and system leaders to measure ROI before deploying systemwide. Loved hearing Rebecca Mishuris outline MGB’s novel approach of piloting two solutions against one another, to pressure test implementation, workflow, and value creation. +++ Proving ROI continues to be challenging: Lots of vendors using lots of different metrics to prove value, but the majority of leaders agreed that estimating the ROI of these new tools today is part science, part art. Demonstrating the ROI of the “soft” metrics around provider burnout and retention seemed to be a big opportunity, especially for investment in more efficient primary care and PCP wellbeing. After all, PCPs drive referrals to specialty-level care (and higher reimbursements) for health systems. Appreciated Anurang Revri's overview of how working team setup can also impact ROI estimates. +++ Value-based care = tools + humanity: VBC will require not only implementing tools to support clinical documentation, risk capture, upcoding, etc, but also humanizing interactions to engage patients. Countless examples, but one that made me smile was a depression focused chat bot responding in the following interaction: - Context: post-discharge check in Bot: Hi Freida, how are you feeling today after your surgery?  Patient: I’m having a tough time, am in a lot of pain.  Bot: Well, it can’t be that bad, I’m sure it will get better with a positive mindset!    🤣 🤣 🤣 Can't wait for next go-around!

Walmart can change the entire tortilla industry, but struggles with health care? Amazon buys One Medical, but will it succeed where others failed? The health care landscape is littered with the wreckage of good intentions from non-healthcare companies. Why? This isn't your typical market. Here's a few reasons businesses struggle in health care: Transaction vs. Relationship: Health care thrives on trust and empathy, not just efficiency (though that matters, too). Can a business built on transactions truly foster healing? Data Chaos: Fragmented data and privacy concerns make it hard to get the insights businesses crave (and often need) to succeed. Treating Diseases, not People: Health care isn't fixing a broken appliance. It's about the whole person, with unique needs and complexities. Algorithms can't replace that human touch. There's a future for businesses in health care, but it requires a fundamental shift. Can they prioritize people over profit? Can they navigate the complexities? We'll see if Amazon or others can crack the code. But one thing's for sure: the current approach isn't working. Let's see who gets it right this time. At the patient level the problem is going to be one of many things so your solution has to cover several issues (technique, behavior, confusion, etc). Should you sell your practice to a VC-Backed MSO Startup? tldr: - new VC-backed startups are forming MSOs to roll up independent practices - The pitch is physicians receive early liquidity from the roll up and are granted ESOP shares that can be used for retirement - Problem is the use of ESOPs with independent practices is not a new concept and has been used with large group practices since the 70's - ESOPs do not grant full ownership and in order for the shares to be valuable for retirement, the practice needs to run the same if not better post roll up - With VC-backed startups being primarily operated by non-physicians with zero clinical or medical practice experience, the likelihood of this happening is extremely low making the entire concept high risk However, there is a viable path for continued ownership + liquidity for independent practices. Walmart is exiting healthcare. This doesn’t surprise me. Walmart was never designed for healthcare, it was designed for retail. Everyone wants a piece of the $4T pie, but it’s a pie full of indigestible ingredients that would make the biggest companies choke. Especially companies that were never built for healthcare. - fee-for-service doesn’t work, especially with very low fees - relying on legacy payors doesn’t work - using insurance for primary care doesn’t work - not paying providers enough doesn’t work - having a clinic next to gardening supplies doesn’t work Throwing millions at healthcare doesn’t automatically work with a large base that comes to you for something else. I believe a properly built and well-executed healthcare startup that “thinks differently”, will eventually move the needle on improving the American healthcare experience. It’s coming, and will surprise everyone.

There are many things we can learn from leading retail brands, such as how to perfect the customer experience that has been refined in brick-and-mortar stores and online in e-commerce sites. However, with the recent decisions of some to leave the #healthcare field, there are also lessons to learn about how our field is different. On the most fundamental level, health care is unlike any other business endeavor. First, it is a mission, not simply a business endeavor. Just as importantly, success is defined by patient health outcomes, a metric that is unique to clinical environments. I welcome the innovation that retail organizations bring, and believe there will be opportunities in the future for traditional health care organizations and new entrants into the market to partner with one another, to find unique solutions to improve patient health outcomes. @wsj (login required)

Are We Beyond Broken? When listening lately to the news - killing of an insurance CEO, major pharmaceutical companies attempting to put the brakes on compounding pharmacies, PBMs so far removed from the original intention to support patients now scooping up small businesses for profit not patients, wars, environmental disasters - one my easily fall into a place of despair and desolation. It is in these times when I pause for a moment to reflect upon the opportunities and gifts that are being presented in the chaos, often called "life in the messy" with my patients! It can be too easy to take the position of being reactive rather than reflective. What is the root of any action? Does it come from a place of greed and desire for power or from a place where we know that we must first take care of ourselves in order to nurture our fellow humans, animals and the precious environment in which we live. It is about the "positive attributes" of reciprocity. Being a part of the healthcare system as well as one of those "complicated patients", I know both sides. In fact, I recently moved with my family, did my online research for a quality hospital system and ugh, very quickly learned that one campus does not speak to the other and messaging my provider is no easy task. I had to educate my new PCP on some specifics about thyroid, female hormones and while he was open to learning, their "EHR system" would not allow him to order my medications that are not listed in their formulary. Luckily, I know how to navigate these challenges and I empower my patients to do the same. The following week, I heard from colleagues how they were told that they are dispensable because after all, "we are ________ hospital system, part of the esteemed ________ medical schools"! Again, I've heard this rumble over the years, feel the anger boiling inside, yet acknowledge that these are man-made systems and institutions desperately in need of transformation. Now back to my original question, "Are we beyond broken?" I do not believe so, but it is time to uncover the cover ups - the hidden money that is being pocketed by select businesses; to look openly at systems that no longer serve; to find the common ground instead of projecting and polarizing. I will continue to be a bridge between both medical worlds, finding the common ground and to be a voice for peace amongst the chaos. It is time to unite as a human collective, supporting life, growth and well being for all.
