We have published the results of the National Engagement on Digital Health and Social Care, a project undertaken in partnership with the Department of Health - Ireland and the Health Service Executive. The project aimed to understand the opinions and attitudes of the Irish public and professionals on a digital approach to health and social care. We surveyed over 2,000 members of the public and over 1,000 professionals working in health and social care. We also held focus groups with members of the public, including representatives from minority groups and people who use disability services, and professionals working in health and social care. The results showed that the public are ready to engage digitally to help manage their health, while professionals think digital care is important to empower patients as partners on their healthcare journey but are concerned that some members of the public may not be ready to interact with them digitally. The findings from this engagement will inform our work in health information, as well as informing the development and provision of digital health systems and services in Ireland by Department of Health - Ireland and Health Service Executive. Community Health Workers (#CHWs) are driving change in underserved communities, and many organizations are collaboratively working to elevate this skilled workforce, including Common Health Coalition, led by Dave A. Chokshi, MD, and coalition members like AHIP, Alliance of Community Health Plans (ACHP), American Hospital Association, and American Medical Association, American Public Health Association, Partners In Health, Visión y Compromiso, and Pear Suite, among others. A recent study from Duke Health researchers highlights the importance of sustainable funding and clearly defined outcomes when addressing patients' health-related social needs (HRSN). The research, published in JAMA Network Open, describes how Duke Health has successfully used NCCARE360/Unite Us to create and track referrals, monitor connections to community-based organizations (CBOs), and measure successful connection rate (SCR) to understand patient receipt of real benefit. 🔑 One Key Finding: An analysis of 115,316 social referrals for 55,000+ unique patients found that successful connection to services was associated with funding and intermediary support for CBOs. For example, "the SCR for the food domain (the only domain to receive continued funding) remained the same" throughout the study period. This important research showcases the value of solutions to financially support our community. Unite Us Payments offers one path where funders can reimburse CBOs at scale for the important services they provide while measuring the success of social care funding initiatives. By supporting the financial sustainability of CBOs and leveraging patient-centered success metrics, we can all partner to meaningfully contribute to the successful integration of health and social care. At Impact Co., we recently had the privilege of collaborating with the Queensland Commonwealth Partnership to develop an evidence-based framework for joint regional governance with the wise counsel of Simon Cotterell and Kathy Hilyard. Regional partnerships and collaboration are often heralded as the future of healthcare reform. Yet, too often, we assume these initiatives will simply 'work' without carefully considering how to design and implement the structures that underpin them. International research tells us that success in this space requires intentional and well-thought-out governance. It’s not just about coming together—it’s about how we come together. This work focused on creating a robust approach to engaging consumers and First Nations stakeholders, ensuring governance drives meaningful, consumer-centred, and place-based health reform. The report has just been published, and we believe it offers valuable insights for anyone committed to advancing sustainable, impactful regional health outcomes. We’re hopeful this framework will be a catalyst for change—not just in Queensland but across Australia. If you’re passionate about driving transformative health reform, I’d encourage you to take a look. Empowering Community Health Workers with Skills in Monitoring & Evaluation Systems. This week, we had the privilege of training an incredible group of Community Health Workers (CHWs) on the essential skills of Monitoring & Evaluation (M&E), data collection, and reporting. These frontline health workers play a critical role in bringing Project interventions and healthcare services closer to the people, especially in underserved communities. Equipping them with data-driven tools and methodologies, empowering them to: 🔸 Track health outcomes effectively 🔹 Collect reliable and accurate data on critical VSLA and health indicators 🔸 Monitor the progress of Saving for health interventions in real-time 🔹 Generate actionable reports to inform policy, improve service delivery, and ensure accountability. With these new skills, CHWs can ensure that health programs not only reach those who need them but are also improved continuously for greater impact. 🌟 Investing in CHWs is an investment in stronger, healthier communities. Together, we are building a future where every decision is backed by accurate data, and every life can be positively impacted through informed health interventions. #CommunityHealth #DataDrivenHealth #MonitoringAndEvaluation #HealthData #HealthcareInnovation #CHWTraining #Healthcare #VSLA #Saving4Health #KaseseDistrict
