Participating in EM JEOPARDY in ESEM 2024 Dubai. (long post alert⏰️) In the fast-paced world of emergency medicine, we are constantly searching for innovative techniques to improve outcomes during critical resuscitations. One such technique gaining traction is the stellate ganglion block (SGB) as an adjunct for managing refractory ventricular tachycardia (VT) during CPR. The stellate ganglion block targets the sympathetic nervous system, reducing adrenergic overdrive—a key driver of arrhythmogenicity in VT. By interrupting this pathway, SGB offers a novel method to stabilize electrical storm cases that do not respond to conventional antiarrhythmic drugs or defibrillation. Emerging evidence from case reports and small series suggests that SGB: 🔹 May reduce the burden of refractory VT episodes 🔹 Could enhance hemodynamic stability 🔹 Is feasible to perform under ultrasound guidance in emergency settings While this technique is not yet routine, its potential impact during resuscitation is undeniable. A large body of evidence from epidemiological and clinical research supports the existence of a strong interconnection between these conditions, such that the unifying term cardio-metabolic-renal (CMR) disease has been defined. This coexistence has remarkable epidemiological, pathophysiologic, and prognostic implications. The mechanisms of hyperglycemia-induced damage to the cardio-renal system are well validated, as are those that tie cardiac and renal disease together.
