📣 Big news: We have expanded our value-based care partnership with Aetna, a CVS Health Company, to cover Aetna Medicare Advantage members with chronic kidney disease (CKD) stages 3b-5 in Arizona, Kansas, Maryland, North Carolina, and Virginia. The extended collaboration aligns with our shared goal of meeting patients upstream before their disease has progressed to kidney failure. Since 2014, Aetna and Interwell have worked together to manage care for patients with end-stage kidney disease as part of a successful value-based care relationship. Under the expanded agreement, eligible members with CKD in five initial states will also have access to holistic, wraparound kidney care and specialized resources from Interwell, including more than 2,100 network nephrologists, renal care coordinators, and in-home virtual support from dietitians, nurses, and social workers. Interwell’s advanced predictive analytics also help identify members at highest risk of progression and in need of additional support.
