Hospitals should increase and improve their care transition activities (CTAs) in order to improve patient outcomes and lower spending, according to a new study in the American Journal of Managed Care.   The study involved more than 400,000 hospitalized Medicare fee-for-service beneficiaries discharged to the community in 2017 and examined the association between hospital-reported CTAs and beneficiaries’ transitional care management (TCM) claims. It found TCM was associated with significantly lower spending, ED visits, hospitalizations, readmissions and 90-day mortality. 🏥 Hospital-at-Home in 2025: Navigating Challenges and Embracing Innovation 🏠 As of late November 2024, CMS has approved 378 hospitals across 140 systems in 39 states for waivers to be reimbursed for hospital-at-home care models. In late December, Congress extended the acute hospital care at home waiver for three months, through March 31, 2025. Without another extension, CMS will stop accepting hospital-at-home applications March 1, 2025. Despite this progress, the temporary nature of these waivers introduces uncertainty, potentially hindering long-term investments in home-based care infrastructure.
