Do you ever wake up in the morning with soreness in your neck, back, or ear or a stiff, tired-feeling jaw? You may be unconsciously grinding your teeth at night. I have patients come into my office with these same complaints as much as once or twice per week. Often, patients who suffer from teeth grinding are burdened with additional symptoms like chronic headaches, difficulty chewing, jaw pain, and sleep issues. As a dentist who specializes in holistic and biological dentistry, I know that in these types of situations, there’s a lot more going on than meets the eye. Teeth grinding is often dismissed as the result of increased stress. Patients think it will go away once their stress is gone. Stress can contribute to teeth grinding but, more often than not, what’s actually happening is that the patient is suffering from a real biological health issue. BioRejuvenation Dentistry is a holistic approach grounded in biological science. Minimally invasive techniques are used to restore the structure and functionality of the dental elements and improve the patient’s overall health. Oral Appliance Therapy: Treatment for teeth grinding starts with relaxing the stressed and spasming muscles in the jaw. To do this, the patient must undergo something called oral orthotic appliance therapy, or MAGO splint therapy. Physical or digital impressions and models of your teeth are used to create your custom MAGO device. I’ll send these impressions to our dental lab where your oral appliance is made. Then, during your follow-up appointment, I’ll fit the device perfectly to your teeth and make any necessary adjustments. If the focus is on treating the sleep apnea a Somnodent device can be utilized to improve airway function during the night. Wearing a SomnoDent device while you sleep allows your lower jaw (mandible) to be moved forward into a comfortable position. This facilitates the relaxation of the tissues at the back of your throat and the chewing muscles that control your jaw. Not only does this treat teeth grinding and clenching, but it can also help with other sleep apnea symptoms. However, my situation was quite different. I had TMJ issues and nighttime bruxism caused by occlusal prematurities. After consulting various professors, I thought I'd have to live with it. Then, a resident suggested orthodontic treatment. Though my teeth seemed fine, there was severe crowding and malocclusion. Orthodontics isn't just about aesthetics; it's about solving real problems and improving oral health. To raise awareness among dentists and patients, we need to emphasize the functional benefits of orthodontic treatment during consultations and educational sessions. Highlighting how correcting malocclusions can prevent issues like TMJ disorders, bruxism, and even digestive problems can shift the focus from aesthetics to overall health. What do eyes, ears and breasts have in common? Lower prices. Allow me to explain… Between 1998 and 2021 the consumer price index for “Hospital and Related Services” has more than tripled. (A 230.4% increase, to be more precise.) That’s a 5.1% increase on average every year since 1998. Overall inflation averaged only 2.1% annually during that same time period. The only other consumer product or service that has increased at the same rate (that I’m aware of) is college tuition, which has increased nearly 5.0% annually since 1998 for four-year public universities. (Which is an entirely different discussion for another day.) Okay, so hospital prices have skyrocketed compared to overall inflation. What if we look at non-hospital medical procedures that aren’t often covered by insurance? Well, we start to find some pretty interesting anomalies. When LASIK eye surgery debuted in the late 1990’s the cost was around $2,500 per eye. Today? $1,500 to $2,500 per eye. That means that even before adjusting for inflation, the cost of LASIK has fallen slightly, or at the very least remained steady. Once you adjust for inflation? LASIK is cheaper than it’s ever been. Another example: every year since 1997, the American Society for Aesthetic Plastic Surgery has issued an annual report on cosmetic procedures in the US, both surgical and nonsurgical. Just like LASIK, 4 of the top 16 plastic surgery procedures have DECREASED in cost, before accounting for inflation. Once you account for inflation, the average price difference across all 16 categories is about half the increase of inflation. (And far, far below the 230% increase of hospital and related services.) Want one more example of the benefits of cash-pay and the price competition it can bring? For years the hearing aid industry has been dominated by expensive devices (ranging from $2,000 - $8,000) that had to be fit by an audiologist. Legislation was just passed allowing for over the counter devices that don’t need to be fit by an expert. We’re still in the early stages but so far there have been a range of devices in the $1,000 range that tend to perform just as well as the more expensive options. Increased innovation and competition is expected to continue. My point with all of this? LASIK, plastic surgery and hearing aids are typically not covered by insurance. For years and years people have been paying cash for these services. Many of these services have also decreased in cost, at the same time many other insurance-covered medical procedures have been skyrocketing. Maybe this means nothing. Or maybe it’s a sign that direct-pay is a more sustainable model for both physicians and patients. What do you think? I’d love to hear your thoughts!
