This is a great overview comparing four different models for Health Information Exchange (HIE) by ⭐️ Christopher SKENE. Personally, I think that the Hub and Spoke approach is best. Why? The strength of this model lies in its alignment with existing healthcare workflows. Healthcare organisations have already established functional (although at times inefficient) networks for sharing patient data based on clinical necessity. These networks, whether operating through traditional or digital channels, exist because they serve real clinical needs between organisations that regularly collaborate on patient care. The Hub and Spoke model builds upon this foundation by: - Digitising existing relationships between healthcare providers - Creating central exchange points within established networks - Connecting these hubs to form a national network - Preserving trusted partnerships while expanding connectivity What distinguishes this approach is its recognition of established healthcare relationships and workflows. Rather than imposing an entirely new system, it enhances existing processes through digital transformation and leverages established clinical relationships. I'm hooked on HTI-2 summaries. Thank you to those willing to unpack the 1067 page rule and summarize for the rest of us, including the amazing #HITLikeAGirl Book Club, where we unpack and discuss the rules. This one is a favorite referencing the highlights and timelines. Some follow-ups for me: - On the adoption of USCDI-4, additional data elements "relevant to behavioral health and marginalized and underserved communities." This is so important, and I support including this in the USCDI, but I want to learn more about how this and other types of sensitive data, can be shared only with authorized people. - Regarding APIs, I am particularly interested in the Prior authorization API. There is a lot of potential there, but there are tough challenges, some of which can be solved with terminologies. - It is exciting to see the CDC moving to FHIR-based data exchange and I look forward to seeing how that gets implemented.
