Back from vacation and finally got to this John Lynn Colin Hung - good stuff! A few thoughts. Good call out on imaging. The ONC heard you - imaging isn't in the EHR which has been the blocker but they are proposing in their new rule that EHRs include links to imaging in patient access and cross-enterprise exchange Regarding volume as a percentage, I have a Cady Heron from Mean Girls quote for you: "The limit does not exist" 1. Interoperability is a superset of distinct problems to be solved. Some are solved or semisolved like e-prescribing. Some are not at all, like referrals. There will always be more as we think broader about actors involved (like SDOH or med device) 2. Each domain can be improved. Changing to new/better standards (generally an exercise in vanity or following trends but sometimes warranted) or new subproblems (e-prescribing is ubiquitous, but pharmacy to pharmacy transfer is not) So in a decade we'll be talking about new interop workflows and upgrading existing digital flows. We see this clearly in the HTI-2 rule. Upgrades to e-prescribing and public health, new birth reporting, new prior auth, and plenty more

Thanks, John Lynn, for hosting me on your podcast. Thanks Stacey Richter for the catch phrase, "Where there is mystery there is margin." The only way we will solve the issues that plague our healthcare system is using information and IT to remove that mystery. The key to accomplishing this will be using #clinicalinformatics to create better data and workflows. If you want more details, listen to the podcast.
