This article in Health Affairs explains the reasons why an extended or permanent waiver for hospital at home care would enhance innovation, concluding that: "Over decades, researchers have developed and tested multiple home-based care delivery models that can now be brought together into a distributed, decentralized health care system that puts the patient at the center of care at home, where they have more agency and power to affect their care. HaH is the keystone of this future home-based care ecosystem. It’s time to claim the future." Read more here: https://lnkd.in/d8n3GNUz #hospitalathome

"It was really a standard that we put in place and now use for all of our patients to make sure that we're identifying great transition points for each patient in the future, and then the whole care team is informed about that," said Dominica Tallarico,B.S., M.S.,FACHE of Allina Health. ➡ Proud to be part of Allina Health's strategy to ensure patients are getting to the sites of care and treatment they need, without the extra strain transitions can put on families. #valuebasedcare #patientcare #healthIT https://lnkd.in/eYFaJ3gR h/t Becker's Healthcare

Expanding acute care #hospital at home can potentially decrease #EmergencyDepartment visits, decrease costs, and keep patients at home to recover. This model allows for patients who meet strict criteria to be treated at home for complaints that often land them in the ED. The company featured in this article, myLaurel™, also aids in transitional care. They have a #paramedic go to the home with a #physician over #telehealth managing the patient virtually. This model makes sense to me. It is a step forward from the basic #telemedicine everyone is used to. It makes use of devices and #RPM we have available. It allows clinicians to expand their practice scope. Best of all, it makes care better for patients, especially those who often get admitted simply because they showed up and had a higher risk profile. This program allows them to get treated at home if their symptoms are deemed safe to do so. Looking forward to hearing the outcomes. This is also yet another plug for telehealth reimbursement permanence - hospital at home codes are needed for programs like these. #digitalhealth #homehealth #seniorcare #healthaccess

This article by Dr. Bruce Leff and David Levine sums up the current state, challenges, and future opportunities of the Hospital at Home (#HaH) Program well. Adoption of the HaH program is ramping up throughout the country, vendors are expanding their scope of work, and studies continue to show positive clinical outcomes and feasible business cases. Regulations need to keep up with the fast pace of innovation in care delivery and make the final decision on the #CMS waiver whether to extend further or make it permanent. Of course, HaH is one of many use cases of Remote Patient Monitoring and Home Care delivery that can potentially have a synergistic effect on outcomes and the bottom line when expended in its scope. #HealthcareonLinkedin #HospitalatHome #Caredelivery #Healthcaredelivery #HealthInnovation #ClinicalInformatics #RPM #HomeCare #Valuebasedcare Link: https://lnkd.in/gJq4hdx4
