READ IT: The fourth spotlight in our series on bridging quality and equity in health care examines the Bridging Research, Accurate Information and Dialogue (BRAID) community engagement model developed by Montefiore Health System, a promising example of a trust-building partnership model between the community and those who provide health care. The spotlight series, the result of a 2022 UHF task force, chronicles how New York health care organizations connect quality and equity in their systems. The task force’s findings were released in a recent UHF report (find it at uhfnyc.org/publications). Over the last year, the health equity committee of OCA-Greater Washington DC (OCA-DC) has been partnering with my research teams at GW School of Medicine & Health Sciences to assess what is known about the health of Asian American, Native Hawaiian, and Pacific Islander (AANHPI) communities in the greater Washington DC region. We found that the majority of data was aggregated and significant gaps in understanding. We subsequently designed this survey with leadership from OCA-DC members to identify key areas of health and healthcare barriers of concern for health disparities in the community. How can we improve Community Based Care? Fostering community-based care requires a collaborative approach that involves healthcare providers, community organizations, local governments, and individuals. To promote community-based care we need to engage the community by 1. Establishing partnerships between healthcare providers, community organizations and local governments. 2. Conducting community need- assessment and identifying health disparities helps in prioritising tasks. 3. Engaging and involving community members in decision-making processes and planning improves the end outcome. Secondly the community should have Care Coordination models... This facilitates communication and collaboration among healthcare providers, social workers and community organizations. Assign specific Care Coordinators and volunteers to facilitate care transitions, providing support and connecting needy patients with community resources. Leverage digital platforms to enhance care coordination, communication and client engagement. Community-Based Services can include 1. Providing home-based care offering medical and social services at patients'homes to reduce hospitalization and improve outcomes. 2. Initiating and establishing community health training programs with effective training modules and programs to enhance knowledge, skills and attitude for effective utilisation of resources. 3. Developing community-based mental health services to provide accessible mental health services, including counseling, group therapy and case based support. 4. Pursue grants, partnerships and innovative financing models to support community-based hospices and health initiatives. 5. Develop business models that ensure long-term sustainability of community-based care programs. At Impact Co., we recently had the privilege of collaborating with the Queensland Commonwealth Partnership to develop an evidence-based framework for joint regional governance with the wise counsel of Simon Cotterell and Kathy Hilyard. Regional partnerships and collaboration are often heralded as the future of healthcare reform. Yet, too often, we assume these initiatives will simply 'work' without carefully considering how to design and implement the structures that underpin them. International research tells us that success in this space requires intentional and well-thought-out governance. It’s not just about coming together—it’s about how we come together. This work focused on creating a robust approach to engaging consumers and First Nations stakeholders, ensuring governance drives meaningful, consumer-centred, and place-based health reform.
