The Community Oncology Alliance has recently raised these issues to Centers for Medicare & Medicaid Services' draft guidance. Dr. Patton: “Like most government programs, it’s complicated, way too complicated. Anything that’s complicated creates a burden on clinics to implement, so there’s going to be an execution burden on practices." "Implementing the policy and tracking whether the community clinic has received the MFP-based reimbursement now will be necessary with the prospective model," Dr. Patton noted. “The IRA implementation policies are complicated, burdensome, and financially unattractive for community practices." I had the chance to speak with Joshua Parry, MD, MS, of DENVER HEALTH MEDICAL CENTER, about results presented at the Orthopaedic Trauma Association Annual Meeting that showed patients with stress-positive lateral compression type 1 pelvic ring injuries who had #fixation were more likely to ambulate without assists at last follow-up and have a lower mortality rate vs. those who did not have surgery. “Right now, the standard of care for these injuries is nonoperative treatment, but I do think some patients, similar to hip #fractures, warrant early fixation and mobilization and that may prevent mortality,” Parry said. Pharmacy Benefit Managers (PBMs) have long been a controversial force in the healthcare system, but their impact on oncology care is drawing sharp criticism. The recently released Oncology Drug Report 2024 highlights troubling trends that reveal how PBMs are complicating cancer treatment, rather than facilitating it. According to the report, 40% of oncologists believe PBMs negatively affect patient care. Why? The PBM practices—like formulary restrictions, prior authorizations, and other bureaucratic hurdles—delay or deny access to critical cancer therapies. PBMs claim to manage costs, but the reality is that their profit-driven models often come at the expense of patients and providers.
