Eastern University Athletic Training Services provides support for each of Eastern’s intercollegiate athletic teams. The sports medicine staff is committed to providing comprehensive, high-quality medical services to every student-athlete. On-site medical services include, but are not limited to, prevention, recognition, evaluation and treatment of athletic injuries and illnesses, rehabilitation services, nutritional counseling, psychological counseling and drug education/counseling.
Eastern's sports medicine staff has four certified athletic trainers, one team physician, a podiatrist and a dentist/oral surgeon. The team physician uses a comprehensive referral source of other specialty-trained physicians, including orthopedic surgeons, neurologists, physiatrists, cardiologists, endocrinologists, gastroenterologists and mental health personnel. A list of these referral services is kept in the sports medicine office. The sports medicine staff also works in conjunction with the health care staff of the Eastern University Health Center and the Cushing Center for Academic Support (CCAS) to meet all the physical and mental health care needs of its intercollegiate student-athletes.
The Eastern University Athletic Training Room, located in the main gymnasium, is staffed by Eastern’s four full-time certified athletic trainers, along with a physical therapist and certified athletic trainer provided by the department’s relationship with NovaCare Rehabilitation. NovaCare Rehabilitation has partnered with Eastern University to provide on-campus physical therapy services to the entire Eastern community.The Athletic Training Room is located in the gymnasium and accessible via the main lobby by going directly across the main gym floor and through the double maroon doors on the far right. Athletic Training Services are coordinated by the Head Athletic Trainer with the assistance of the Assistant Athletic Trainers who administer care under the supervision of licensed physicians. These services are available to all intercollegiate student-athletes. Hours of operation are posted outside the doors.

Important Change to Medical Papework for Student Athletes.
ALL PAPERWORK SHOULD BE SUBMITTED THROUGH THE EASTERN HEALTH PORTAL
This is new. Every student-athlete must fill out the necessary paperwork before competition or practice sessions. Look in your Eastern Email for an email from John Post.
Students who intend to compete in 2023-24 must fill out the necessary medical paperwork. Download and use the correct version of the form for your status. All students who are first year student-athletes or transfers into Eastern should download
this form. Once the proper information and signatures are on the documents, they should be uploaded to the Eastern Health Portal. Be certain to fill out all the relevant documents in the Eastern Health Portal.
Returning student athletes also need to download and fill out a form. This form can be downloaded
here. Once again, every document in the health portal must be filled out and
this document must be completed and uploaded.

Before a student-athlete will be allowed to participate in any team activities (e.g., practices, competitions, conditioning and/or weight training), that individual must be medically cleared and have the appropriate medical and insurance information paperwork on file in the sports medicine office. Eastern University Department of Athletics requires all student-athletes wishing to compete on an intercollegiate sports team to obtain physical examination clearance from an appropriately qualified medical physician before to reporting for their respective season.
Proof of insurance is also required before a student-athlete is permitted to participate in any team activity. Those who have been diagnosed with and/or are being treated for Attention Deficit Hyperactivity Disorder (ADHD), are required to fill out an ADHD medical exception form as well. The NCAA mandates that all student-athletes know of their Sickle Cell Trait status.
Please download these forms, complete all of them as instructed and mail to the Eastern University Department of Athletics by the appropriate due date --- August 1 for all fall sports and August 15 for winter and spring sports. Please be advised that incomplete information regarding medical information and/or insurance information may result in delay of clearance; therefore, it is in your best interest to complete these forms carefully and without omission.
***NOTE*** --- In regards to the physical, the NCAA requires that all student-athletes have a Pre-Participation Examination (PPE) completed no more than six months prior to their first participation (practice, competition, or out-of-season conditioning activities). (Bylaw 17.1.5 of Division III manual)
